a) Two copies of the tentative parcel map;
b) 100' Owners List;
c) 100' Occupant List;
d) Mailing Labels (one set);
e) 100' Radius Map;
f) Check #3203 in the amount of $14.08 for postage; and
g) Original, signed property owner list certification form.
Per your request, I have also included a copy of the Preliminary Report.
