To address post-development pollutants that may be generated from development projects, the City requires that new
development and significant redevelopment priority projects incorporate Permanent Storm Water Best Management
Practices (BMP's) into the project design per the City's Standard Urban Stormwater Management Plan (SUSMP). To
view the SUSMP, refer to the Engineering Standards (Volume 4, Chapter 2) at INWW'.carlsbadca.goWstandards.
Initially this questionnaire must be completed by the applicant in advance of submitting for a development application
(subdiVision, discretionary permits and/or construction permits). The results of the questionnaire determine the level of
storm water standards that must be applied to a proposed development or redevelopment project. Depending on the
outcome, your project will either be subject to 'Standard Stormwater Requirements' or be subject to additional criteria
called 'Priority Development Project Requirements'. Many aspects of project site design are dependent upon the
storm water standards applied to a project
Your responses to the questionnaire represent an initial assessment of the proposed project conditions and impacts.
C,ity staff has responsibility for making the final assessment after submission of the development application. If staff
determines that the questionnaire was incorrectly filled out and is subject to more stringent storm water standards than
initially assessed by you, this Will result in the return of the development application as incomplete. In this case, please

AT&T proposes to install (3) panel antennas inside existing faux rooftop chimneys. Also, (2)
equipment cabinets will be added to the existing equipment area. The project will also include
the installation of(6) RRU's, (6) TMA's, and (1) GPS antenna.
The antennas will be installed inside (3) existing faux chimneys. No alteration to the size or
appearance of the chimneys is proposed.
The installation will be a modification of an existing unmanned wireless telecommunications
facility; the purpose of this modification is to provide enhanced mobile broadband service (L TE
technology) operating in the 704-746 MHz spectrum (as part of the AT&T 4G network) for the
surrounding area.
Maintenance of the facility includes approximately monthly site visits, which are conducted
within standard daytime working hours. There are no generators or additional air conditioning
units proposed for the installation. No additional lighting is proposed.
Site Justification:
The project site is located within an area zoned 'P-C' and is used as a residential dwelling, and
currently has an approved wireless telecommunications facility (Permit # CUP 00-36). As this
site is fully screened, it meets the stealth criteria specified by Council Policy 64.
As this project is a modification to an existing site, no alternative candidates were considered.

Tel net, Inc performed an RF computational analysis of the RF environment surround-
ing the facilities installed by AT&T at this location. The facility is located on a Chim-
ney.
AT&T is licensed by the Federal Communications Commission ("FCC") to provide
wireless -communications services. As required by the FCC, wireless system opera-
tors perform an assessment of the potential human exposure to radio frequency
emissions emanating from transmitting antennas at the site.
Antenna specifications presented herein are based on direct evidence from informa-
tion from the site manager or building manager, information from the licensees, edu-
cated estimates by the field technician or a combination of some or all of these
sources.
1.2 Statement of Compliance
A site is considered in compliance with FCC regulations if there are areas that ex-
ceed the FCC exposure limits and there are no RF hazard mitigation measures in
place. Any carrier which has an installation that contributes more than 5% of the a p-
plicable MPE must participate in mitigating these RF hazards. Per AT&T's corporate
policy, the FCC's general population limits are applicable to all rooftop sites, regard-
less of the level of access control. As presented in the sections below, based on
worst-case predictive modeling, the worst-case emitted power density may exceed
the FCC's general public limit.

Since AT&T contributes more than 5% of the MPE, should this site be non-compliant
for any reason, all other operators who contribute greater than 5% would all be liable
to bring the site into compliance.

1.3.1 lockoutJTagout Procedures for Antenna, Transmission line and Power
Amplifier Maintenance
Whenever anyone is working on an antenna, transmission line, high power am-
plifier (HPA), or multi-channel power amplifier (MCPA), the transmitter (power
amplifier) MUST be turned off. This can be accomplished either locally by flipping
a circuit breaker(s) or remotely by command from the NMC/NOC.
The person initiating or requesting the transmitter shutdown is the ONLY person
authorized to restore the transmitter to service. This person is responsible for
making sure that ALL work has been completed, that ALL cables have been
properly reconnected, and that EVERYONE is clear of the work area before the
transmitter is reactivated. Generally, this person is considered to be the one ac-
tually performing the work. In the case of a contractor working at an active site,
the FElT echnician may initiate the request on behalf of the contractor.

After securing permission to shut the transmitter down, the Field Engineer
(FE)/Field Technician (FT) will turn off the circuit breaker and verify that the cor-

over the circuit breaker(s) to prevent accidental activation by an unauthorized
person and place a TAG on, or in the immediate vicinity of, the circuit breaker(s).
The tag should state "Do Not Operate." At the NMC/NOC the same note, includ-
ing date and time and location, must be entered in the computer or a tag must be
placed on the monitor frame in such a manner that the console operator will be
made aware that the transmitter can not be activated without permission from the
person who initiated the maintenance request.
The FEIFT will turn the key(s) over to the person performing the work. Upon
completion of the work, this person performing the task will return the key(s). As
a precautionary measure, prior to r~activating the transmitter, the FE/FT MUST
verify, to the extent possible, that all connections have been made and that the
work area is clear of personnel.
1.3.3 LockoutJTagout Procedure, Remote Shutdown
After requesting the NMC/NOC to shut the transmitter down, the FEIFT will verify
that the correct transmitter was deactivated. The FEIFT will then place a TAG on
or in the immediate vicinity of transmitter. The tag should state "Do Not Operate."
At the NMC/NOC the same note, including date/time, must be entered in the

1.5 RF Modeling
The modeling calculations assume that the antennas are operating at 100% capacity;
that all antenna channels are transmitting simultaneously and that the radio transmit-
ters are operating at full power. Obstructions (trees, buildings etc) that would normal-
ly attenuate the signal are not taken into account. As a result, the predicted signal le-
vels are more conservative (higher) than the actual signal levels will be from the
measurement conclusions.

2 Antenna Inventory
The Antenna Inventory shovvs all transmitting antennas on the site (see Table 1 ).
This inventory was verified on site and was used by T elnet to perform software mod-
eling of RF emissions . The inventory coincides with the site diagrams on this report,
identifying each antennas location at the site.
For other carriers at the site, the use of "Generic" as an antenna model, or " Un-
known" for an operator means the information with regard to the carrier, their FCC li-
cense and I or antenna information was not available nor could it be secured while on
site. Equipment, antenna models and nomina\ transmit power were used for model-
ing, based on past experience with radio service providers. ·
Relevant administrative and compliance-related information about the antenna site
rooftop area is summarized in the table below:

3 Modeling Summary and Assumptions
3.1.1 General Model Assumptions
In this report, it is assumed that all antennas are operating at full power at all times. Soft-
ware modeling was performed for all transmitting antennas located on the site. Telnet, Inc
has further assumed a 100% duty cycle and maximum radiated power.
The site has been modeled with these assumptions to show the maximum RF energy
density. Telnet Inc believes this to be a worst case analysis, based on best available data.
If at any time power density measurements were to be made, Telnet Inc believes the real
time measurements would indicate levels below those shown in this report. By modeling in
this way, we have conservatively shown exclusion areas (areas not to be entered without
a personal RF monitor, carriers reducing power or performing real time measurements to
show real time exposure levels}.
3.1.2 Use of Generic Antennas
For the purposes of this report, the use of 'Generic' as an antenna model, or 'Unknown' for
a wireless carrier, means that the information about the carrier, their FCC license and/ or
antenna information was not provided and could not be obtained while on site. In the event
of unknown information, Telnet will use our industry specific knowledge of equipment, an-
tenna models and transmit power to model the site. If more specific information can be ob-
tained for the unknown measurement criteria, remodeling of the site is recommended. If
no information is available regarding the transmitting service associated with an unidenti-
fied antenna, using the antenna manufacturer's published data regarding the antenna's
physical characteristics makes more conservative assumptions.

5 FCC Limits for MPE
The FCC guidelines for human exposure to RF electromagnetic fields were derived
from the recommendations of two expert organizations, the National Council on
Radiation Protection and Measurements ("NCRP") and the Institute of Electrical and
Electronics Engineers ("IEEE"). The exposure guidelines are based on thresholds for
known adverse effects and they incorporate appropriate margin of safety. The federal
health and safety agencies such as: the Environmental Protection Agency ("EPA"),
the Food and Drug Administration ("FDA"}, the National Institute on Occupational
Safety and Health {"NlOSH") and the Occupational Safety and Health Administration
("OSHA") have also been actively involved in monitoring and investigating issues
related to RF exposure.
The FCC's MPE limits are based on exposure limits over a wide range of frequencies
recommended by the NCRP and the exposure limits developed by the IEEE and
adopted by the American National Standards Institute ("ANSI") to replace the 1982
ANSI guidelines. The limits for localized absorption are based on the
recommendations of both the ANSI/IEEE and the NCRP. The potential hazard
associated with the RF electromagnetic fields is discussed in OET Bulletin No. 56
"Questions and Answers about the Biological Effects and Potential Hazards of RF
Electromagnetic Fields". This document can be obtained on the FCC website at
http://www.fcc.gov.

4 Analysis and Computation
Based on emission patterns of the antennas at this location most of the energy
emitted is spread towards the horizon. This assumes the antennas have a zero
downtilt. If a mechanical downtilt other then zero is applied to the antennas then the
maximum energy emitted will need to be calculated using the information below.
The following formulas can be used for calculating the power density.
Power density is calculated by dividing the surface area of the sphere or the unit area
normal to the direction of the propagation. This information is usually shown in units
of microwatts per square centimeter (uW/cm2), milliwatt per square centimeters
(mW/cm2), or watts per square meter (W/m2).

7 Glossary of Terms
1. Electromagnetic Field (energy density)-the electromagnetic energy contained in an
infinitesimal volume divided by that volume.
2. Exposure -Exposure occurs whenever and wherever a person is subjected to
electric, magnetic or electromagnetic fields other than those originating from
physiological processes in the body and other natural phenomena.
3. General Population I Uncontrolled Exposure-applies to human exposure to RF
fields when the general public is exposed or in which persons who are exposed as a
consequence of their employment may not be made fully aware of the potential for
exposure or cannot exercise control over their exposure. Therefore, members of the
general public always fall under this category when exposure is not employment-
related.
4. Maximum Permissible Exposure (MPE) -the rms and peak electric and magnetic
field strength, their squares, or the plane-wave equivalent power densities associated
with these fields to which a person may be exposed without harmful effect and with
an acceptable safety factor.
5. Occupational I Controlled Exposure-applies to human exposure to RF fields when
persons are exposed as a consequence of their employment and in which those

At the December 7, 2011 Planning Commission meeting, your application was considered. The
Commission voted 7-0 to approve your request. The decision of the Planning Commission will become
final on December 17, 2011.
The time within which judicial review of this decision must be sought is governed by Code of Civil
Procedure, Section 1094.6, which has been made applicable in the City of Carlsbad by Carlsbad

This early public notice is to let you know that a development application to add three

to the existing Wireless Communication Facility is within your neighborhood and has
been submitted to the City of Carlsbad on February 24, 2011. The project application is
undergoing its initial reviews by the City.

PROJECT DESCRIPTION: AT&T is proposing to add three (3) additional panel

Land Development Engineering has completed its review of the above referenced project for
application completeness and has determined that the application and plans submitted for this
project are complete and suitable for continued review. Engineering staff does not have any
comments to add to the project. Please add the following conditions to the approving
resolution/letter:
1. Developer shall comply with the City's Stormwater Regulations, latest version, and shall
implement best management practices at all times. Best management practices include
but are not limited to pollution treatment practices or devices, erosion control to
prevent silt runoff during construction, general housekeeping practices, pollution
prevention and educational practices, maintenance procedures, and other management
practices or devices to prevent or reduce the discharge of pollutants to stormwater,
receiving water or stormwater conveyance system to the maximum extent practicable.
Developer shall notify prospective owners and tenants of the above requirements.

Thank you for applying for Land Use Permits in the City of Carlsbad. The Planning Division has
reviewed your Conditional Use Permit Amendment, application no. CUP 00-36x1(A), as to its

The application is incomplete, as submitted. Attached are two lists. The first list is information which
must be submitted to complete your application. The second list is project issues of concern to staff.
In order to expedite the processing of your application, the "incomplete" items and your response to
the project issues of concern to Staff must be submitted directly to your staff planner; therefore,

Please submit verification that the WCF is in compliance with FCC Guidelines. The
developer/operator shall submit to the Planning Director either verification that the WCF is
categorically excluded from having to determine compliance with the guidelines per 47 CFR
Section 1.1307(b)(1) or a project implementation report that provides cumulative field
measurements of radio frequency (RF) electromagnetic fields of all antennas installed at the
subject site. The report should quantify the RF emissions and compare the results with
currently accepted ANSI/IEEE standards as specified by the FCC.
Please provide an 8 % x 11" search ring map for your RF coverage objectives. The exhibit
should include the boundaries that encompass the search ring for the proposed WCF site.
This information will be used to help staff identify, if any, alternative sites not yet analyzed by
the applicant that are listed as "preferred locations" within City Council Policy No. 64.
Please provide justification for adding more antennas to an existing "non-preferred" site.
Include a written description of the site selection process undertaken for the additional
antennas, coverage objectives, and reasons for rejecting any other "preferred" sites in the
area.
ISSUES OF CONCERN
Planning:
G Although the proposed addition to a WCF is located in a discouraged location (Residential
zone) as listed in Location Guideline A.2.b. of City Council Policy No. 64, the City's Wireless
Communication Facilities Guidelines, staff may support this site/location if no other feasible
alternative exists in a "preferred" zone. There is currently not enough information provided
within the application to make a conclusive determination at this time.
In addition to the street light located at the corner of La Costa Avenue and Cadencia Street,
please include the use of existing poles (e.g. street lights), substantially similar replacement
poles in the same location, or vertical structures already located in the right-of-way along La
Costa Avenue as alternative locations for the proposed WCF to meet RF coverage
objectives. Public right-of-way of roads adjacent to residential zones and identified on the
Circulation Plan contained in the Circulation Element of the City's General Plan are listed as
"preferred locations" for the placement of WCF's in CC Policy No. 64 (A.1.j).
Similarly, there is an electrical transmission line corridor north of the proposed project, off of
Cadencia Street, that maybe a suitable alternative site.
Engineering: ,
~Developer shall comply with the City's Stormwater Regulations, latest version, and shall ~implement best management practices at all times. Best management practices include but
are not limited to pollution treatment practices or devices, erosion control to prevent silt
runoff during construction, general housekeeping practices, pollution prevention and
educational practices, maintenance procedures, and other management practices or devices
to prevent or reduce the discharge of pollutants to stormwater, receiving water or stormwater
conveyance system to the maximum extent practicable. Developer shall notify prospective
owners and tenants of the above requirements.

Planning Items Needed to Complete the Application
1. See included RF Study.
2. See included search ring map. Note that as the project scope entails only the modification of existing sites (see below), the search
ring shown on the map does not indicate that other sites in the area were considered as alternatives to modifying the existing site.
The search ring shown indicates a general area which could potentially serve to address the coverage needs for this site, based upon
geographic location and proximity to adjacent existing sites within the network.
3. The submitted application which proposes to add additional antennas to the existing permitted site is part of AT& T's larger efforts
to upgrade its network with L TE (Long Term Evolution) technology through the addition of new antennas and equipment to existing
sites. The LTE technology being implemented serves to deliver through the network enhanced mobile broadband service to AT&T
customers. As the LTE technology upgrade is being implemented as an upgrade to the existing sites in AT&T's network, with
extremely limited exception, the project does not include the exploration or development of new facilities within the San Diego
market.
In addition to upgrading the existing site with L TE technology, the additional antennas will serve to provide improved coverage and
capacity within the service area. Please refer to the included coverage maps. See below for additional discussion regarding the site
selection. Note that the addition of antennas does not require the expansion or visible alteration of the existing faux concealment
chimneys, and would result in no impact on the surrounding community.
Planning Issues of Concern
1. While the existing site is located in a "discouraged location" as defined by Council Policy 64, it should be noted that the Council
Policy 64, Location Guidelines item 4 states, "Collocating with existing or other planned wireless facilities is recommended whenever
feasible." Also, Council Policy 64, Design Guidelines item 3 states, "Whenever feasible and appropriate, WCF design and placement
should promote and enable collocation." As the existing site is a permitted telecommunications facility, utilizing the existing site
through modification of the antennas and equipment best meets the directive regarding collocation. Furthermore, as the existing
facility is completely stealthed per Council Policy 64 Design Guidelines, and the proposed modification does not in any way expand
or otherwise alter the exterior appearance of the facility, use of the existing site through modification would best meet the directive
regarding stealth design.
The entire area surrounding the existing site, (and almost the entire southeastern portion of the City of Carlsbad) is zoned as either
residential or open space, which are discouraged locations. As demonstrated in the included search ring map and coverage maps,
there are no acceptable locations in the vicinity which would be considered a preferred location (see below for discussion of utilizing
the surrounding public right-of-way and utility transmission corridor. It should be noted that there are no opportunities for locating
this site in a more preferred location than existed at the time the site was originally submitted for review and approved.
2. The suggestion to consider use of any exgg street lights or oth~r public right-of-way ~allation appears to be extremely
misleading. Historically, the City of Carlsbad has not supported efforts by any carrier to locate antennas on new or existing vertical
elements in the public right-of-way, despite the provision in Council Policy 64 identifying these areas as preferred locations. There is
no new technology available which would allow this installation to be better concealed and in conformance with staff's
interpretation of Council Policy 64 stealthing requirements than has been previously presented for consideration. With the proposed
installation of nine antennas for this facility, it is inconceivable that any proposed design would receive staff support.
3. Regarding the transmission corridor to the north of the existing site: There are three existing SDG&E steel transmission towers
located within this corridor and in reasonable proximity to the search ring shown in the justification map. The first tower, located
approximately 1200 feet northwest of Cadencia Street, would be an inferior location to the existing site, due to its base elevation
being approximately 40 feet lower than that of the existing facility, which would result in a decrease in coverage ability.
With both towers located approximately 465 and 1295 feet respectively to the east/southeast of Cadencia Street, there would likely
be problems with situating the equipment cabinets within the available corridor space. There exists a second set of utility lines to
the south of the SDG&E lines. With the requirement that all equipment be located more than ten feet from the drip line of all
overhead transmission lines, (and accounting for the access road which is also an unusable area) there is insufficient room within the
corridor at these locations available for locating the equipment. Furthermore, locating the site at either of these towers would
require extensive trenching in order to provide power and telecommunications to the site, which would create significant
disturbance of the existing landscape and environment. Lastly, the utilization of any of location within the transmission corridor
would be unjustifiable in that it would overlook the opportunity to utilize collocation at an existing approved facility and would
result in the unnecessary proliferation of telecommunications facilities in the area.
Engineering Issues of Concern

needs to become 50% bigger. I would like to understand the city's position on this project as well.
I am very interested -since this facility predates the city's policies around where cell towers can be
located -to see what are the parameters for assessing how big a site like this located in a residential
area can be built out. I'd also like to know if there are any other residential sites this big in Carlsbad,
in San Diego County, or in California. I think this is important information and that the City should
collect and document this as part of this application and public record.
