SUBJECT: CUP 0139(A) - ST ELIZABETH SETON CATHOLIC CHURCH
This is a courtesy notice. , In a recent audit of our records, we found that CUP 0139(A) - ST
ELIZABETH SETON CATHOLIC CHURCH, located at 6628 Santa Isabel St, will expire on
March 2, 2004. Should you, the property owner, require an extension of this permit, please
send the following:
J Written request for an extension;
J Completed Disclosure Statement (enclosed);
J Land Use Application (enclosed);
J A check in the amount of $595;
J Additional information will be requested by the planner.

Your request for an extension must be received before December 5, 3003, to allow adequate
time to process your application and schedule a hearing before the Planning Commission. If you do not request an extension by this date, the permit will be considered invalid and to continue operating, you will be required to submit an application for a new permit, along with a
check in the amount of $2,950.
