SUBJECT: CUP 146(B) -CHURCH OF JESUS CHRIST LATTER DAY SAINTS
This is a courtesy notice. In a recent audit of our records, we found that CUP 146(B) -
CHURCH OF JESUS CHRIST MITER DAY SAINTS, located at 1975 Chestnut Avenue, will expire on September 6, 2005. Should you, the property owner, require an extension of this
permit, please send the following:
J Written request for an extension;
J Completed Disclosure Statement (enclosed);
J Land Use Application (enclosed);
J A check in the amount of $685;
J Noticing Fee in the amount of $285;
J Additional information will be requested by the planner.

Your request for an extension must be received before June 2, 2005, to allow adequate time to
process your application and schedule a hearing before the Planning Commission. If you do not
request an extension by this date, the permit will be considered invalid and to continue
operating, you will be required to submit an application for a new permit, along with a check in
the amount of $3,395.
