Dear Anne Hysong:
Per your request please find enclosed a newly completed disclosure
statement, and a copy of the 1993 water quality sample results
collected by the California Dept. of Health Services, Environmental
Branch.
I have not been mailing these reports to the engineering department as required by the conditions of my CUP because someone in the
Carlsbad Engineering Department indicated that they really had no
use for them, and that he was aware that I had them on file should
he ever need them. We both agreed that keeping track of these
reports was just needless paperwork and postage. (unfortunately I
do not remember who it was that instructed me to proceed in this
manner.) I think that eliminating this reporting requirement would
be in the best interest of both parties. I will always have them
available on file as submitted to me by the DHS and FDA.
Please consider this letter my request to eliminate or modify that requirement when my new CUP is issued.
