SUBJECT: CU 194BX2 -AQUACULTURE SYSTEMS
In a recent audit of our records, we found that CUP 194Bx2, expired on May 6, 1998. The City
is offering a one-time grace period which will allow you to retroactively extend the CUP, if you
act by October 11 2001. The Conditional Use Permit runs with the land, and requires.the
property owner to request an extension of this permit. In order to begin the process, please send
the following:

J Completed Disclosure Statement (enclosed);

Written request for an extension;
Mailing labels (instructions enclosed); and
A check in the amount of $595.
Send the required documents and warrants to:

Your request for an extension must be received before October 11 2001 If you do not
request an extension by this date, the permit will be considered invalid and to continue operating,
you will be required to submit an application for a new permit, along with a check in the amount of $2,950.
