SUBJECT: (CUP 85(C)) - DISCLOSURE STATEMENT - PILGRIM UNITED
CHURCH OF CHRIST CONDITIONAL USE PERMIT
Dear Jim:
I am sending you the second page of the required disclosure statement for the subject
project for your signature. An authorized member of the Board of Trustees must sign .
as the owner. I have the Board of Trustees list of names, however, please attach to the d
signed disclosure a letter authorizing you or one of the other board members to sign on
the Pilgrim Church’s behalf. I have also faxed the disclosure to Dana Whitson for her ,
signature as applicant. She previously signed as the owner, however, since she is not
one of the Board of Trustees, I cannot accept her signature.
Please return the signed disclosure statement and letter of authorization to me by May
26, 1999 so that we can proceed to public hearing before the Planning Commission on
June 16,1999.
