This is a courtesy notice to inform you that the Conditional Use Permit (CUP) for CUP 93-0l(A)xl-Prime Auto Center,
located at 6021 Paseo Del Norte, will expire on November 4, 2013. Should you, the property owner, wish to extend
this permit, you will need to submit a complete application (applications and forms may be obtained at
http://www.carlsbadca.gov/services/departments/planning/Pages/applications.aspx or our front counter) consisting
of the following:
y' Written request for an extension;
y' Completed Disclosure Statement;
y' Land Use Review Application;
y' Development Permits form;
y' A check in the amount of totaling all application fees; and
y' One complete set of plans (may be copies of those already approved).
Once we receive and review your application, we may request more information to complete the application.
Please submit the required documents and fees to:

Your request for an extension must be submitted before the permit expiration date of November 4, 2013; otherwise
the permit will be considered invalid and to continue operating, you will be required to submit an application for a
new permit, along with a check for all fees associated with initiating a new permit. Please be advised that in the event
your permit expires and you subsequently apply for a new permit, the application would again be subject to the full
discretionary review process of the designated decision-making authority (Planning Commission and/or City Council).
This means the application may be approved, approved with conditions, or denied based on the review process and
findings of the decision-making authority.

SUBJECT: CUP 93-01 (A)x1-PRIME AUTO CENTER
On November 14, 2006 staff completed a yearly review of the conditions of approval for your
Conditional Use Permit (CUP 93-01 (A)x1) for the Prime Auto Center.
Staff has determined that Conditions No. 5, 6 and 7 of Resolution No. 5480 (attached) have not
been met; therefore, the CUP is in violation of the conditions of approval and could be revoked.
However, staff would like to work with you to correct this oversight. Please review the attached
resolution and submit a Storm Water Management Plan (SWMP) to the Engineering
Department for their review. Please respond to this letter by December 8, 2006 or staff will
schedule this CUP for an upcoming public hearing by the Planning Commission to resolve the
non-compliance issue.

CAPITAL LEITERS, left justified, void of any punctuation. For any address
other than a single-family residence, an apartment or suite number must be
included but the Apartment, Suite and/or Building Number must NOT appear in
the street address line. DO NOT type assessor's parcel number on labels. DO
NOT provide addressed envelopes -PROVIDE LABELS ONLY. Acceptable
fonts are: Arial 11 pt, Arial Rounded MT Bold 9 pt, Courier 14 pt, Courier New 11
pt, and MS Line Draw 11 pt. Sample labels are as follows:
