SUBJECT: CUP 94-04x2 - CARLSBAD SD475
Our records indicate a letter was sent to you on June 1, 2009, advising you that your
Conditional Use Permit (CUP) No. 94-04x2 was due to expire on September 20, 2009.
To date, we have not received a response. We are enclosing a copy of the letter
previously sent. If we do not hear from you by August 3, 2009, there may no longer be
adequate time to process your application and schedule a hearing before the Planning
Commission prior to expiration of your CUP. Should this occur, your CUP will become
null and void and the usage will be required to cease.

SUBJECT: CUP 94-04x2 - CARLSBAD SD475
This is a courtesy notice. In a recent audit of our records, we found that CUP 94-04x2, located
at 1015 Chestnut Avenue, will expire on September 20, 2009. The City has revised the
Conditional Use Permit regulations to create a Minor Conditional Use Permit that is acted on by
the Planning Director. In order to continue operating this conditional use, approval of a Minor
Conditional Use Permit is required. Please submit the following:
> Land Use Application (enclosed);
> Completed Disclosure Statement (enclosed);
> A check in the amount of all application fees;
> Plans and information listed on the application checklist (enclosed).

Your request for a Minor Conditional Use Permit must be received before July 1, 2009, to allow
adequate time to process your application. If you do not request a Minor Conditional Use
Permit by this date, the previously approved permit will be considered invalid and you will need
to cease operating the conditional use or reapply for a new Minor Conditional Use Permit.
