To view the SUSMP, refer to the Engineering Standards (Volume 4, Chapter 2) at www.carlsbadca.gov/standards. Initially this questionnaire must be completed by the applicant in advance of submitting for a development application (subdivision, discretionary permits and/or construction permits). The results of the questionnaire determine the level of storm water standards that must be applied to a proposed development or redevelopment project. Depending on the outcome, your project will either be subject to 'Standard Stormwater Requirements' or be subject to additional criteria called 'Priority Development Project Requirements'. Many aspects of project site design are dependent upon the storm water standards applied to a project. Your responses to the questionnaire represent an initial assessment of the proposed project conditions and impacts. City staff has responsibility for making the final assessment after submission of the development application. If staff determines that the questionnaire was incorrectly filled out and is subject to more stringent storm water standards than initially assessed by you, this will result in the return of the development application as incomplete. In this case, please make the changes to the questionnaire and resubmit to the City. If you are unsure about the meaning of a question or need help in determining how to respond to one or more of the questions, please seek assistance from Land Development Engineering staff. A separate completed and signed questionnaire must be submitted for each new development application submission. Only one completed and signed questionnaire is required when multiple development applications for the same project are submitted concurrently. In addition to this questionnaire, you must also complete, sign and submit a Project Threat Assessment Form with construction permits for the project. Please start by completing Section 1 and follow the instructions. When completed, sign the form at the end and submit this with your application to the city. Examples: single family homes, multi-family homes, Automotive repair shop. A facility categorized in any one of Standard Industrial Classification (SIC) codes 5013, 5014, 5541, 7532-7534, and 7536-7539 Restaurants. Any facility that sells prepared. foods and drinks for consumption, induding stationary lunch counters and refreshment stands selling prepared foods and drinks for immediate consumption (SIC code 5812), where the land area for development is greater than 5,000 square feet. In general, these indude all projects that corllribute to an exceedance to an impaired water body or which create new impervious surfaces greater than 5000 square feet and/or irllroduce new landscaping areas that require routine use offertilizers and pesticides. In most cases Unear pathway projects that are for infrequent vehide use, such as emergency or maintenance access, or for pedestrian or bicyde use, are not considered Pollutant-generating Development Projects if they are built with pervious surfaces or if they sheet flow to surrounding pervious surfaces. Unenforceability of the lien of the insured mortgage because of the inability or failure of the insured at Date of Policy, or the inability or failure of any subsequent owner of the indebtedness, to comply with the applicable doing business laws of the state in which the land is situated. 5. Invalidity or unenforceabillty of the lien of the Insured mortgage, or claim thereof, which arises out of the transaction evidenced by the insured mortgage and is based upon usury or any consumer credit protection or truth in lending law. 6. Any claim, which arises out of the transaction vesting in the insured the estate of interest insured by this policy or the transaction creating the interest of the insured lender, by reason of the operation of federal bankruptcy, state insolvency or similar creditors' rights laws. EXCEPTIONS FROM COVERAGE -SCHEDULE B, PART I This policy does not insure against loss or damage (and the Company will not pay costs, attorneys' fees or expenses) which arise by reason of: 1. Taxes or assessments which are not shown as existing liens by the records of any taxing authority that levies taxes or assessments on real property or by the public records. Any claim of invalidity, unenforceability or lack of priority of the lien of the Insured Mortgage as to advances or modifications made after the Insured has Knowledge that the vestee shown in Schedule A is no longer the owner of the estate or interest covered by this policy. This proposed AT&T telecommunication site will utilize directional transmit panel antennae configured in three (3) sectors. The antennae for all sectors are planned to be mounted to a mono-eucalyptus with their center at least 39.0 feet above grade directed at 40 (sector A), 140 (sector B) and 260 (sector C) degrees true north. The antennas specified are Kathrien model #800-10765 for all sectors. Technical specifications of these antennae are provided in attachment two. The sectorized antennas are designed to transmit utilizing LTE, GSM and UTMS technology with an effective radiated power (ERP) of up to 1,116 watts per sector within a bandwidth between approximately 734 and 894 MHz (collectively referred to in this report as cellular frequencies) and with an ERP of up to 2,080 watts per sector within a bandwidth between approximately 1,945 and 1,980 MHz (collectively referred to in this report as PCS frequencies). There are also two other existing wireless carriers (T-Mobile and Sprint/Nextel) that are located on the same property but at different locations than the AT&T facility. The other carriers's site design specifications are also depicted in attachment one. The maximum cumulative RF exposure from all three carriers is provided in this report. Calculation Methodology, Results & Recommendations Calculations were made in accordance with the recommendations contained in the Federal Communications Commission, Office of Engineering and Technology Bulletin 65 (edition 97-01, page 24, equation 10) entitled "Evaluating Compliance with FCC-Guidelines for Human Exposure to Radiofrequency Electromagnetic Fields." Several assumptions were made in order to provide the most conservative or "worse case" projections of power densities. Calculations were made assuming that all channels were operating simultaneously at their maximum design effective radiated power. Attenuation (weakening) of the signal that would result from surrounding foliage or buildings was ignored. Buildings can reduce the signal strength by a factor of 10 (i.e., 10 dB) or more depending upon the construction material. The ground 1 or other surfaces were considered to be perfect reflectors (which they are not) and the RF energy was assumed to overlap and interact constructively at all locations (which they would not) thereby resulting in the calculation ofthe maximum potential exposure. In fact, the accumulations of all these very conservative assumptions will significantly overestimate the actual exposures that would typically be expected from such a facility. However, this method is a prudent approach that errs on the side of safety. The maximum public RF exposure from this AT&T facility was calculated to be less than 1.5% of the FCC public safety standard. Exposure details are shown in appendix A. Details regrading the T -Mobile and Sprint/Nextel facilities were not available but a conservative estimate of there RF exposure can be made from the site plans provided and my extensive experience analyzing similar RF site designs for these carriers. The maximum contribution to potential public exposure in the area surrounding the AT&T facility from the T -Mobile and Sprint/Nextel facilities will be less than 0.1% of the public safety standard. Thus the maximum cumulative exposure from three carriers in the area surrounding the AT&T facility will be less than 1.6% of the public safety standard. A sign conforming to with ANSI C95.2 color, symbol and content, and other markings as appropriate, should be placed close to the antennas with appropriate contact information in order to alert maintenance or other workers approaching the antenna to the presence of RF transmissions and to take precautions to avoid exposures in excess of FCC limits. RF Safety Standards The two most widely recognized standards for protection against RF field exposure are those published by the American National Standards Institute (ANSI) C95 .1 and the National Council on Radiation Protection and measurement (NCRP) report #86. The NCRP is a private, congressionally chartered institution with the charge to provide expert analysis of a variety of issues (especially health and safety recommendations) on radiations of all forms. The scientific analyses of the NCRP are held in high esteem in the scientific and regulatory community both nationally and internationally. In fact, the vast majority of the radiological health regulations currently in existence can trace their origin, in some way, to the recommendations of the NCRP. All RF exposure standards are frequency-specific, in recognition of the differential absorption ofRF energy as a function of frequency. The most restrictive exposure levels in the standards are associated with those frequencies that are most readily absorbed in humans. Maximum absorption occurs at approximately 80 MHz in adults. The NCRP maximum allowable continuous occupational exposure at this frequency is 1,000 uW/cm2 • This compares to 5,000 uW/cm2 at the most restrictive ofthe PCS frequencies (~1,800 MHz) that are absorbed much less efficiently than exposures in the VHF TV band. The traditional NCRP philosophy of providing a higher standard of protection for members of the general population compared to occupationally exposed individuals, prompted a two-tiered safety standard by which levels of allowable exposure were substantially reduced for "uncontrolled " (e.g., public) and continuous exposures. This measure was taken to account for the fact that workers in an industrial environment are typically exposed no more than eight hours a day while members of the general population in proximity to a source of RF radiation may be exposed continuously. This additional protection factor also provides a greater margin of safety for children, the infirmed, aged, or others who might be more sensitive to RF exposure. After several years of evaluating the national and international scientific and biomedical literature, the members of the NCRP scientific committee selected 931 publications in the peer-reviewed 2 scientific literature on which to base their recommendations. The current NCRP recommendations limit continuous public exposure at PCS frequencies to 1,000 uW/cm2 . The 1992 ANSI standard was developed by Scientific Coordinating Committee 28 (SCC 28) under the auspices of the Institute of Electrical and Electronic Engineers (IEEE). This standard, entitled "IEEE Standards for Safety Levels with Respect to Human Exposure to Radio Frequency Electromagnetic Fields, 3 kHz to 300 GHz" (IEEE C95 .1-1991 ), was issued in April 1992 and subsequently adopted by ANSI. A revision of this standard (C95.1-2005) was completed in October 2005 by SCC 39 the IEEE International Committee on Electromagnetic Safety. Their recommendations are similar to the NCRP recommendation for the maximum permissible exposure (MPE) to the public for PCS frequencies (950 uW/cm2 for continuous exposure at 1,900 MHz) and incorporates the convention of providing for a greater margin of safety for public as compared with occupational exposure. Higher whole body exposures are allowed for brief periods provided that no 30 minute time-weighted average exposure exceeds these aforementioned limits. On August 9, 1996, the Federal Communications Commission (FCC) established a RF exposure standard that is a hybrid of the current ANSI and NCRP standards. The maximum permissible exposure values used to assess environmental exposures are those ofthe NCRP (i.e., maximum public continuous exposure at PCS frequencies ofl,OOO uW/cm2 ). The FCC issued these standards in order to address its responsibilities under the National Environmental Policy Act (NEPA) to consider whether its actions will "significantly affect the quality of the human environment." In as far as there was no other standard issued by a federal agency such as the Environmental Protection Agency (EPA), the FCC utilized their rulemaking procedure to consider which standards should be adopted. The FCC received thousands of pages of comments over a three-year review period from a variety of sources including the public, academia, federal health and safety agencies (e.g., EPA & FDA) and the telecommunications industry. The FCC gave special consideration to the recommendations by the federal health agencies because of their special responsibility for protecting the public health and safety. In fact, the maximum permissible exposure (MPE) values in the FCC standard are those recommended by EPA and FDA. The FCC standard incorporates various elements of the 1992 ANSI and NCRP standards which were chosen because they are widely accepted and technically supportable. There are a variety of other exposure guidelines and standards set by other national and international organizations and governments, most of which are similar to the current ANSI/IEEE or NCRP standard, figure one. The FCC standards "Guidelines for Evaluating the Environmental Effects of Radiofrequency Radiation" (Report and Order FCC 96-326) adopted the ANSI/IEEE definitions for controlled and uncontrolled environments. In order to use the higher exposure levels associated with a controlled environment, RF exposures must be occupationally related (e.g., wireless company RF technicians) and they must be aware of and have sufficient knowledge to control their exposure. All other environmental areas are considered uncontrolled (e.g., public) for which the stricter (i.e., lower) environmental exposure limits apply. All carriers were required to be in compliance with the new FCC RF exposure standards for new telecommunications facilities by October 15, 1997. These standards applied retroactively for existing telecommunications facilities on September 1, 2000. The task for the physical, biological, and medical scientists that evaluate health implications of the RF data base has been to identify those RF field conditions that can produce harmful biological effects. No panel of experts can guarantee safe levels of exposure because safety is a null concept, and negatives are not susceptible to proof. What a dispassionate scientific assessment can offer is the presumption of safety when RF-field conditions do not give rise to a demonstrable harmful effect. 3 Summary & Conclusions The cumulative RF exposure from both the proposed AT&T and the existing T-Mobile and Sprint/Nextel wireless telecommunications facilities as specified above will be in full compliance with FCC RF public safety standards. Wireless telecommunications transmitters, by design and operation, are low-power devices. Even under maximal exposure conditions in which all the channels from all antennas, for all three carriers, are operating at full design basis power, the maximum cumulative exposure will not result in RF exposures in excess of 1.6% of the public safety standard at any publically accessible location. This maximum exposure is more than 62 times lower than the FCC public exposure standards for these frequencies. A chart of the electromagnetic spectrum and a comparison ofRF power densities from various common sources is presented in figures two and three respectively in order to place exposures from wireless telecommunications systems in perspective. It is important to realize that the FCC maximum allowable exposures are not set at a threshold between safety and known hazard but rather at 50 times below a level that the majority of the scientific community believes may pose a health risk to human populations. Thus the previously mentioned maximum exposure from the site represent a "safety margin" from this threshold of potentially adverse health effects of more than 3,100 times. Given the low levels of radiofrequency fields that would be generated from this facility, and given the evidence on biological effects in a large data base, there is no scientific basis to conclude that harmful effects will attend the utilization ofthe proposed wireless telecommunications facility. This conclusion is supported by a large numbers of scientists that have participated in standard-setting activities in the United States who are overwhelmingly agreed that RF radiation exposure below the FCC exposure limits has no demonstrably harmful effects on humans. These findings are based on my professional evaluation of the scientific issues related to the health and safety of non-ionizing electromagnetic radiation and my analysis of the technical specification as provided by AT&T. Product Overview The ABS 6301 is a main-remote RBS and a member of the RBS 6000 family. The RBS 6301 consists of an outdoor main unit and a number of Remote Radio Units (RRUs). An optical fiber cable and DC cable connects each RRU to the main unit, as shown in Figure 1. Optical fiber cables are available in standard lengths from a few meters up to several hundred meters The fiber network can also be extended or built using leased fiber networks. In this case the extension and delay of the fiber network must be considered. The equivalent supported distance between the main unit and RRU is for WCDMA 40 km when using RRUW or RRUS 01, and 15 km when using RRU22. For L TE and GSM, consult the market representative Figure 1 RBS 6301 For WCDMA the RBS can be configured with up to three sectors with up to four carriers. For GSM the RBS can be configured with up to three sectors with up to 'I 6 carriers and for LTE the RBS can be configured with up to three sectors. A I 201 RBS Description 2.1 The number of RRUs supported depends on the available radio configurations. The configurations currently available are described in RBS Configurations. Note: Not all features are supported by all RBS configurations. Current RBS configurations can be found in RBS Configurations. It is not necessary for basic RBS functions. The equipment is located outside the main unit. Optional equipment located inside the main unit is described in Section 4 on page 17. 2.2.1 ASC, TMA, RETU, and RIU The following mast-located units are placed close to the antenna: • Tower Mounted Amplifier (TMA) • WCDMA Antenna System Controller (ASC) • L TE or WCDMA Remote Electrical Tilt Unit (RETU) • L TE or WCDMA RET Interface Unit (RIU) The TMA and the ASC are uplink amplifiers and improve the Receiver (RX) sensitivity. The RETU enables remote tilt of the antenna system. An ASC or a RIU is required to enable the RBS to communicate with the RETU. The RBS supports Antenna Interface Standards Group (AISG) 2.0/3GPP. 2.2.2 Battery Backup The external batteries are connected to a DC filter (PCF) on the RBS. The maximum cable length between the RBS and the external batteries is 15 m total. 2.2.3 ESB TG synchronization is the technology used to expand one RBS cabinet with another RBS cabinet in the same cell for GSM. The External Synchronization Bus (ESB) is the cable connected between the DUs that support TG synchronization. 2.2.4 GBF The main unit can be installed on a single Generic Base Frame (GBF). More information about the GBF can be found in Section 3.2.2 on page 9. 53/IS5HZA 6001 A 3 ASS Description 2.2.5 GPS The RBS can be connected to a Global Positioning System (GPS), which is used for timing synchronization and positioning of the RBS. 2.2.6 Installation Frame The main unit can be installed on an installation frame that can be attached to a pole or a wall. A client can be connected to the DU for configuration and service purposes through the RBS EM. In GSM, Site LAN communicates with the Operation and Maintenance Terminal (OMD. Position E -Optical RRU Interface The connection between the main unit and the RRU is made by an optical cable through an Supported Small Form-factor Pluggable (SFP) module. Optical fiber cables are available in standard lengths .from a few meters up to several hu ndrad meters For longer distances between the main unit and an RRU and for greater connection flexibility in general, an existing optical fiber transmission network can be used to increase the total length of the optical cable. Optical cables are connected to the optical fiber transmission network by a connection box. The al1ematives with their corresponding cable types are described in Section 6 on page 36. Position G-ESB Interface (Optional} TG synchronization is used to expand one RBS cabinet with another RBS cabinet in the same cell for GSM. The External Synchronization Bus (ESB) is the cable connected between the DUs that support TG synchronization. Position H -Service Outlet Interface (Optional) The service outlet is provided with a 1 0 A two-pole circuit breaker and Residual Current Breaker (RCB). The maximum current available in the service outlet is 8 A. Only double-insulated equipment is allowed to be connected to the service outlet at temperatures below -25°C. This is because the built-in RCB functions cannot be guaranteed at temperatures below -25"C. The service outlet is optional. The power cable is recommended to be multi-stranded and have a cross-sectional area between 16 mm2 and 95 mm2• More detailed information on cross-sectional cable areas and fuse sizes can be found in Section 3.5 on page 12. The connection interface is shown in Figure 23. Parallel AC mains and DC main feed is not supported. Incoming AC power including protective earth are connected by screw terminals in the PCU AC. The terminals accepts cables with an area between 2.5 mm2 and 16 mm2. The external battery power Is connected through the PCF to the BFU and the RBS internal power. The battery backup interface is shown in Figure 24. The E1, T1, or J1 connection is made through terminal blocks without screws on the OVP, situated at the bottom of the cabinet. The OVP accept cables with an area of 0.1 -1.5 mmZ. Two different OVP types are available; one for 100-120 n, and one for 75 n. The transmission cable is routed from the OVP to the DU. The E1, T1, and J1 connection interface in the DU can be equipped with two female RJ-45 connectors and occupies positions ETA and ET B. The SAU can handle up to 32 external alarms. Alarms ports are also provided by most RRUS, see the corresponding RRU documentation. Note: These RRU alarm ports are currently supported in LTE and WCDMA Single Mode node configurations. An alarm can be generated by two alarm conditions: • Closed loop condition, called Normally Open (NO) NO means that an alarm is triggered when an open switch is closed. • Open loop condition, called Normal Closed (NC) NC means that an alarm is triggered when a closed switch is opened. NC is the default alarm condition. The customer can configure the alarm condition. More information on the SAU can be found in SAU Description. Acoustic Noise Emission Note: Acoustic noise measurements will be available In a later release of this document. This section will contain information about acoustic noise emission from an RBS. Values will be given for a free-standing RBS as well as for an RBS placed against a wall. Note: The acoustic noise emission values for a free-standing RBS and an RBS installed against a wall will be tested according to the EN ISO 9614·2 standard. The values will be calculated in accordance with the EN ISO 11203 standard. The values can vary depending on the relative sound absorbency of the installation environment. Table 16 and Table 17 will show the sound pressure level at different ambient temperatures, directions and distances from the RBS. That the requested use is not detrimental to existing uses or to uses specifically permitted in the zone in which the proposed use is to be located in that the proposed WCF is located within a preferred location (Office Zone) as listed in Location Guideline A.l.c. of City Council Policy Statement No. 64, and all aspects of the WCF exhibit stealth design and are completely hidden within the proposed faux mono-eucalyptus tree and new screened equipment enclosure and the WCF use is not precluded by the project site's Governmental Facilities (G) land use designation. Furthermore, the proposed WCF will result in only minimal additional building coverage, only minor alterations to the existing site design are required to accommodate a new ground mounted equipment enclosure which is constructed of tan colored slump block walls and is fully screened by new landscaping, and the proposed antennas will be mounted on a faux mono-eucalyptus tree that will blend in with the new and existing trees and landscaping on the site. The project has been conditioned to comply with FCC RF Exposure Guidelines. 3. That the site for the proposed conditional use is adequate in size and shape to accommodate the yards, setbacks, walls, fences, parking, loading facilities, buffer areas, landscaping and other development features prescribed in this code and required by the City Planner in order to integrate the use with other uses in the neighborhood in that the 78 inch tall panel antennas are located within and painted to match the color of a faux mono-eucalyptus tree thereby exhibiting a stealth design and the equipment enclosure will be constructed with tan colored slump block walls and will be adequately screened from view with new landscaping. 4. That the street system serving the proposed use is adequate to properly handle all traffic generated by the proposed use in that the WCF use generates very little traffic, requiring only routine monthly maintenance visits and occasional visits in response to operational problems. In addition, the existing street system is adequate to properly handle any traffic generated by the use. 5. That the City Planner has determined that the project belongs to a class of projects that the State Secretary for Resources has found do not have a significant impact on the environment, and it is therefore categorically exempt from the requirement for the preparation of environmental documents pursuant to Section 15303 (New Construction of Small Structures) of the state CEQA Guidelines. In making this determination, the City Planner has found that the exceptions listed in Section 15300.2 of the state CEQA Guidelines do not apply to this project. 6. The City Planner has reviewed each of the exactions imposed on the Developer contained in this resolution, and hereby finds, in this case, that the exactions are imposed to mitigate impacts caused by or reasonably related to the project, and the extent and the degree of the exaction is in rough proportionality to the impact caused by the project. Conditions: 1. Approval is granted for MCUP 13-05 as shown on Exhibits "A"-"T" dated October 29, 2013 on file in the Planning Division and incorporated herein by reference. No vested rights are gained by Developer or a successor in interest by the City's approval of this Minor Conditional Use Permit. 3. Staff is authorized and directed to make, or require the Developer to make, all corrections and modifications to the MCUP 13-05 documents, as necessary to make them internally consistent and in conformity with the final action on the project. Development shall occur substantially as shown on the approved Exhibits. Any proposed development different from this approval shall require an amendment to this approval. 4. The Developer/Operator shall and does hereby agree to indemnify, protect, defend and hold harmless the City of Carlsbad, its Council members, officers, employees, agents, and representatives, from and against any and all liabilities, losses, damages, demands, claims and costs, including court costs and attorney's fees incurred by the City arising, directly or indirectly, from (a) City's approval and issuance of this Minor Conditional Use Permit, (b) City's approval or issuance of any permit or action, whether discretionary or non-discretionary, in connection with the use contemplated herein, and (c) Developt:!r/Operator's installation and operation of the facility permitted hereby, including without limitation, any and all liabilities arising from the emission by the facility of electromagnetic fields or other energy waves or emissions. 5. The Developer shall comply with all applicable provisions of federal, state, and local ordinances in effect at the time of building permit issuance. 6. This project shall comply with all conditions and mitigation measures, which are required as part of the Zone 11 Local Facilities Management Plan and any amendments made to that Plan prior to the issuance of building permits. 7. MCUP 13-05 shall be reviewed by the City Planner on a yearly basis to determine if all conditions of this permit have been met and that the use does not have a substantial negative effect on surrounding properties or the public health, safety and general welfare. If the City Planner determines that: 1) the minor conditional use permit was obtained by fraud or misrepresentation; or 2) the use for which such approval is granted is not being exercised; or 3) the conditions of approval have not been met; or 4) the minor conditional use permit is being or recently has been exercised contrary to any of the terms or conditions of approval; or 5) the use for which such approval was granted has ceased to exist or has been suspended for one year or more; or 6) the use is in violation of any statute, ordinance, law or regulation; or 7) the use permitted by the minor conditional use permit is being or has been so exercised as to be detrimental to the public health, safety or welfare or so as to constitute a nuisance, the City Planner shall hold an informal public hearing and aft1er providing the permittee the opportunity to be heard, the City Planner may revoke and terminate the minor conditional use permit in whole or in part, reaffirm the minor conditional usE! permit, modify the conditions or impose new conditions. MCUP 13-05-NS0028 OMWD OPS CENTER October 29, 2013 Page 4 8. This Conditional Use Permit is granted for a period of 10 years from October 2, 2013 through October 28, 2023. This permit may be revoked at any time after a public hearing, if it is found that the use has a substantial detrimental effect on surrounding land uses and the public's health and welfare, or the conditions imposed herein have not been met. This permit may be extended for a reasonable period of time not to exceed ten (10) years upon written application of the permittee made no less than 90 days prior to the expiration date. The Planning Commission may not grant such extension, unless it finds that there are no substantial negative effects on surrounding land uses or the public's health and welfare. If a substantial negative effect on surrounding land uses or the public's health and welfare is found, the extension shall be denied or granted with conditions which will eliminate or substantially reduce such effects. There is no limit to the number of extensions the Planning Commission may grant. 9. Prior to the issuance of a Building Permit, owner/applicant shall submit to the City a Notice of Restriction to be filed in the office of the County Recorder, subject to the satisfaction of the City Planner, notifying all interested parties and successors in interest that the City of Carlsbad has issued a Minor Conditional Use Permit on the real property owned by the owner/applicant. Said Notice of Restriction shall note the property description, location of the file containing complete project details and all conditions of approval as well as any conditions or restrictions specified for inclusion in the Notice of Restriction. The City Planner has the authority to execute and record an amendment to the notice, which modifies or terminates said notice upon a showing of good cause by the owner/applicant or successor in interest. 10. This approval shall become null and void if building permits are not issued for this project within 18 months from the date of project approval. 11. Developer shall submit and obtain City Planner approval of a Final Landscape and Irrigation Plan showing conformance with the approved Preliminary Landscape Plan and the City's Landscape Manual. Developer shall construct and install all landscaping as shown on the approved Final Plans, and maintain all landscaping in a healthy and thriving condition, free from weeds, trash, and debris. 12. The first submittal of Final Landscape and Irrigation Plans shall be pursuant to the landscape plancheck process on file in the Planning Division and accompanied by the project's building, improvement, and grading plans. 13. Developer shall report, in writing, to the City Planner within 30 days, any address change from that which is shown on the permit application. 14. Developer/Operator shall comply with the Federal Communication Commission's guidelines on limits for human exposure to radio frequency (RF) electromagnetic fields. Within six (6) months after the issuance of occupancy, and with any time extension or amendment request, the Developer/Operator shall submit to the City Planner either (1) verification that the project is categorically excluded from having to determine compliance with the RF exposure guidelines per 47 CFR §1.1307(b)(1); or (2) a project implementation report which provides cumulative field measurements of RF emissions of all antennas installed at the subject site. The report shall quantify the RF emissions and compare the results with the exposure limits established by the FCC guidelines. Developer shall cause property owner to apply for, execute, and submit to the City Engineer for recordation, an Encroachment Agreement covering private landscaping, telecommunication equipment and enclosure located over existing public right-of-way or easements as shown on the site plan. Developer shall pay processing fees per the city's latest fee schedule. Said encroachment agreement is not necessary if the ~Underlying easements are vacated to the satisfaction of the city engineer prior to the issuanct! of building permits. 17. Developer shall comply with the city's Stormwater Regulations, latest version, and shall implement best management practices at all times. It appears that the redlined hatched area will be on a slope and if so it will be required to meet slope revegetation requirements (i.e. straw mat, Baccharis ground covers and Acacia shrubs), The Prunus will need to remain as shown for required screening. If the slope is to be graded differently, please provide a grading plan that documents this change for cross checking. Please resolve and revise water use calculations and work sheets as appropriate.
