Dear Gary:
Please find the attached documents, Exhibits A thru K, as formal submittal for the
Amendment of the Planned Industrial Permit #86-6 of the Opus Plaza Project. This Amendment is consistent with the original approved P.I.P. Plan and is to
include Phase I1 construction, a three (3) story multi-tenant office building. The
following is a log of the contents of those exhibits to assist you during your
review:

I have also included with this submittal, the Executed Public Facilities Agreement, the Envioronmental Impact Assessment Form, the Title Report, and the Disclosure
Statement as required.
You will find the adjustment to the daylight elevation as originally noted on the
civil engineering drawings dated September 8, 1988, has been changed to indicate
maintaining the existing elevation as per our discussion on January 4, 1989.
Further, it is our understanding based on your conversation with Dennis Cornelius
on Friday, January 6, 1989, that processing of this Amendment and concurrent site
review will occur within a three (3) week time period. Due to our pending
structural steel order for the building, I hereby request that we be advised
immediately of any potential delays.
Please do not hesitate to call our office if you are in need of additional
information or if I may assist you in any way. Thank you for your attention to
this most urgent matter.

1. Five (5) copies of site plan (24" x 36" folded to 8-1/2" x 11") shall
include the following information:
- Name and address of applicant, owner, engineer and/or
architect, etc., and their registration number.
- All easements.
- Dimensioned location of: - Access, both pedestrian and vehicular, showing service areas
and points of ingress and egress, offstreet parking and loading
areas showing location, number and typical dimension of
spaces, and wheel stops.
- Distances between buildings and/or structures.
- Building setbacks (front, rear and sides).
- Location, height, and materials of walls and fences.
- Location of freestanding signs.
- All driveways, alleys and streets (with street names) to scale
on adjacent and across the street properties for a distance of
100 feet beyond the limits of subject site.
- Existing curbs, gutters, sidewalks and existing paving widths
within 100 feet on adjacent and across the street properties.
- Watercourses.
- Typical street sections.
- Existing contours and proposed contours in 2' intervals.
- Any existing median island within 100 feet of subject site
nearest cross streets on both sides with plus or minus distances
from subject site.
- Drainage facilities and/or flow lines.
- Location of all buildings within 100 feet of subject
properties.
- Grading quantities.
- A vicinity map showing major cross streets.
- Existing utilities within 100 feet of subject properties.

11") shall include the following information:
- Landscape zones per the City of Carlsbad Landscape
Guidelines Manual. - Typical plant species and their sizes for each planting zone. - An estimate of the yearly amount of irrigation
(supplemental) water required to maintain each zone. - Landscape maintenance responsibility (private or common)
for all areas. - Percent of site used for landscaping.

4. One (1) copy of 8-1/2" or 11" location map (suggested scale 200" -
vicinity maps on the site plan are not acceptable).
5. Environmental Impact Assessment Form ($175).
6. Public Facility Agreement: 2 copies; one (1) notarized Original and
(1) reproduced copy.
7. Disclosure Statement.
8. Preliminary Title Report (current within the last six months).
