Enclosed is the City's "Notice Concerning Aircraf& Environmental Impacts" form which you
previously signed and notarized. In processing the fom. for recordation, our City Attorney
realized that information needed to record the document was missing from our fom. We have
added this information and we are hereby requesting that you initial these changes. The changes
include (1) adding "a California general partnership" after Hamann Investment on page one and
(2) adding "HAMANN INVESTMENT, a California partnership" above your signature and
"Jeffrey C. Hamann, a general partner" below your signature on page three.
Unless this new information is incorrect, please write your initials next to each added text as
indicated on the attached "post-it" paper and return to my attention. We have enclosed a self
addressed envelope for your convenience. I apologize if these changes have caused any
