APPLICATION FORM, AND THE APPLICATION FEE.

Both signs areas are identical and constructed of .5” acrylic painted blue (logo) and black (letters), stud mounted on
building fascia.
The dimensions of the sign (including the logo and the letters) is 72”X36” (WXH).
The sign will be mounted in the center of the area designated Sign Area #I and Sign Area #2 as shown on the front
and side view drawing.
