C. Source of Illumination.
D. Proposed sign copy.

APPLICATION FORM, AND THE APPLICATION FEE.

SITE PLAN REQUIREMENT FOR POLE, MONUMENT, AND FREESTANDING SIGN
APPLICATIONS
The following example illustrates the information that is required for all pole, monument, and
freestanding sign permit applications. Prior to approval, all such proposed signs must be reviewed
for potential issues by the Traffic Engineering Department, which will not allow signs to be
approved over the counter. Additional time will be required for on-site inspection.

Relationship to Streets: Signs shall be designed and located so as not to interfere with the
unobstructed clear view of the public right-of-way and nearby traffic regulatory signs of any
pedestrian, bicyclist or motor vehicle driver

Dear Mr. Doty:
The Architectural Review Committee (ARC) of the Carlsbad Research Center (CRC) has
reviewed the plans subniitt-ed for installation of a single monument sign on Lot 43 of the CRC.
The plans are entirely consistent with the Design Guidelines as they apply to signage.
Therefore your plans are considered APPROVED.
Once you obtain a sign permit from the City of Carlsbad, you are free to have the sign installed
per the approved plans. Please mail a copy of your sign pennit to:

We are in receipt of Covidien's submittal to the Architectural Review Committee of the
Carlsbad Research Center for changes to the exterior monument sign at the above
referenced building.
In addition to the Architectural Review Committee's approval, in accordance with lease
paragraphs 7.3 & 53, the exterior monument sign requires Lessor's approval. At this
time, we approve the changes to the monument sign. We expect the existing
monument sign to be restored to its original condition upon lease temnination.
Therefore, please advise your sign company to take caution when placing your new
aluminum sign on the exterior ofthe exiting concrete sign.
