Provide an elevation for all proposed sign(s) which specifies the following:
A. Dimensions and area for all existing and proposed sign(s).
B. Materials the sign(s) will be constructed of.
C. Proposed sign copy.

Dear Samantha,
Enclosed please find a copy of your sgnage stamped "Pm-Apmwed as Noted" by Landlord. This approval
is subject to a signed Lease Agreement between Tenant and Landlord. The sign contractor is proceeding at
their own risk and expense. Signage approval is subject to all required governmental approvals. Please be
aware that prior to installation of any signage at the above-referenced facility, Tenant must provide our offices
with a copy of the sign permit and a certificate of insurance fnxn Tenant's sign contractor naming additional
insureds as per the attached list. Note, channel letters sign request is approved as noted however, under canopy shop drawings must be submitted to our offices no later than October 8,1999. Also, under
canopy sign must be installed by October 29,1999. If these items are not met by the required time
period than signage is to be removed at Tenant's andor sign contractor sole cost and expense.
