RE: Current Insurance Certificate for Sidewalk Sign
Dear Sir/Madam:
Our records indicate that you were issued Permit #01-01 for a Sidewalk Sign that is
located within the public right-of-way. An ongoing requirement of the permit is that you
maintain a valid certificate of insurance for the item within the public right-of-way
naming the City of Carlsbad and the Carlsbad Redevelopment Agency as additional
insured.
This is a courtesy notice to inform you that we received a Notice of Cancellation for your
certificate of insurance we currently have on file. The Cancellation Notice is effective
November 13, 2003. If you would like to continue to have the Sidewalk Sign within the
public right-of-way, please have your insurance carrier forward to us an updated
insurance certificate within 10 business days ofthe expiration date. Otherwise, the item
must be removed fi'om the public right-of-way.

Dear Sir or Madam:
Our records indicate that you were issued Permit #01-01 for an A-Frame Sign that is
located in the public right-of-way. A requirement of the permit is that the City of
Carlsbad and the Carlsbad Redevelopment Agency be listed as Additional Insured on
your insurance policy. The most recent certificate of insurance on file expires or expired
on (no certificate on file). If the certificate of insurance on file has not yet expired,
please ask your insurance provider to ensure that a new certificate is sent to us upon
renewal. If there is no certificate of insurance on file or if it has expired, please submit a
current certificate of insurance within ten davs (10). If a current certificate of insurance
is not received by the Carlsbad Redevelopment Agency, your permit may be revoked
and/or the A-Frame Sign may be confiscated by Code Enforcement.

Re: Approval of Sidewalk Sign Permit
Dear Shah:
Thank you for submitting your application for a Sidewalk Sign Permit. The revised sign plan for
the requested sidewalk sign was received from your sign manufacturer on January 11, 2001.
Having received all required documents your application has been approved. Attached is a
copy of the approved Sidewalk Sign Permit for your records.
The attached permit allows you to retain your sign for; 1) as long as the Housing and
Redevelopment Commission allows sidewalk signs to remain, 2) as long as the sign continues
to meet the standards outlined within the Carlsbad Village Master Plan and Design Manual, and
3) as long as the sign is consistent with this permit approval. It is important to note that the
Housing and Redevelopment Commission will complete an annual review of the sidewalk sign
regulations to determine their continued applicability. A valid Certificate of Insurance is required
for as long as the sidewalk sign continues to be placed within the public right-of-way.
