The following supplemental information must accompany the completed application, according to project type.
1. ALL PROJECTS, EXCLUDING SIGNS:
E] A completed Environmental Impact Assessment Form - Part I.
[X] The following items must be provided once it has been determined that the application is complete:
Radius Map, Two separate sets of the Property Owners List and Labels. The property owner list must
be typed and contain the names and addresses of all property owners within a 300 feet radius of the
subject property (including the applicant and/or owner). The list shall include the San Diego County
Assessor's parcel number from the latest assessment rolls. Except for single family residences, the
apartment or suite number must be included on the address mailing labels for property owners within
300 feet radius of subject property; the parcel number should not be typed on the address labels. The
radius map must be provided with a scale not less than 1" = 200' showing each lot within 300 feet of the
exterior boundaries of the subject property.

10. FINDINGS AND CONDITIONS OF APPROVAL (IF APPLICABLE) Page 1 of 2
The following findings have been made in order to approve the Administrative Review Permit for
property located at 2892 Jefferson Street:
1. The Housing and Neighborhood Services Director has determined that the project is consistent
with the policies, goals and action programs set forth within the Carlsbad General Plan.
2. The Housing and Neighborhood Services Director has determined that the project is consistent
with all applicable development standards and land use policies set forth within the Village
Master Plan and Design Manual and Village Redevelopment Plan.
3. The Housing and Neighborhood Services Director has determined that the project is exempt from
the requirements of the California Environmental Quality Act (CEQA) per Section 15301(e)(1) of
the State CEQA Guidelines as a minor addition to an existing structure which will not have an
adverse significant impact on the environment.

The following conditions have l|^n approved for the subject project:
1. This approved permit serves as the required Administrative Review Permit for the subject
project. The applicant has received approval to construct a 907 square foot (697 square feet of
net floor area) addition to be used as office space, which shall replace two existing structures on
the site consisting of the same amount of net floor area.
2. The project shall be completed according to, and consistent with, the plans approved by the
Housing and Neighborhood Services Director on the date on Page 1. The approved plans have
been stamped by the Housing and Neighborhood Services Director and indicate the approval
date of ^/27///.
3. Complete construction plans prepared by a licensed architect, must be submitted to the Building
Division for pian review.
4. This permit shall be subject to revocation if the applicant fails to conform to any of the above
conditions of approval.
5. Developer shall submit to the city planner, a reproducible 24" x 36", mylar copy of the Site Plan
reflecting the conditions approved by the final decision making body. The reproducible shall be
submitted to the city planner, reviewed and, if acceptable, signed by the city's project engineer
and project planner prior to submittal of the building plans, improvement plans, grading pians, or
final map, whichever occurs first.
6. Prior to hauling dirt or construction materials to or from any proposed construction site within
this project, developer shall apply for and obtain approval from, the city engineer for the
proposed haul route.
7. This project is approved upon the express condition that building permits will not be issued for
the development of the subject property, unless the district engineer has determined that
adequate water and sewer facilities are available at the time of permit issuance and will continue
to be avaiiabie until time of occupancy.
Fees/Agreements
8. Prior to approval of any grading or building permits for this project, developer shall cause owner
to give written consent to the city engineer for the annexation of the area shown within the
boundaries of the site plan into the existing City of Carlsbad Street Lighting and Landscaping
District No. 1 and/or to the formation or annexation into an additional Street Lighting and
Landscaping District. Said written consent shall be on a form provided by the city engineer.
9. Developer shall pay traffic impact and sewer impact fees based on Section 18.42 and Section
13.10 of the City of Carlsbad Municipal Code, respectively. The Average Daily Trips (ADT) and
floor area contained in the staff report and shown on the site plan are for planning purposes
only.
10. Developer shall comply with the city's Stormwater Regulations, latest version, and shall
implement best management practices at all times. Best management practices include but are
not limited to poiiution control practices or devices, erosion control to prevent silt runoff during
construction, general housekeeping practices, pollution prevention and educational practices,
maintenance procedures, and other management practices or devices to prevent or reduce the
discharge of pollutants to stormwater, receiving water or stormwater conveyance system to the
maximum extent practicable. Developer shall notify prospective owners and tenants of the above
requirements.
11. Developer shall complete and submit to the city engineer a Project Threat Assessment Form
(PTAF) pursuant to City Engineering Standards. Concurrent with the PTAF, developer shall also
submit the appropriate Tier level Storm Water Compliance form and appropriate Tier level Storm
Water Pollution Prevention Plan (SWPPP) as determined by the completed PTAF all to the
satisfaction of the city engineer. Developer shall pay all applicable SWPPP plan review and
inspection fees per the city's latest fee schedule.
12. Developer shall incorporate measures with this project to comply with Standard Stormwater
Requirements per the city's Standard Urban Stormwater Management Plan (SUSMP). These
measures include, but are not limited to: 1) reducing the use of new impervious surfaces (e.g.:
paving), 2) designing drainage from impervious surfaces to discharge over pervious areas (e.g.:
turf, landscape areas), 3) and designing trash enclosures to avoid contact with storm runoff, all
to the satisfaction ofthe city engineer.
Dedications/Improvements
13. Developer shall install a new 6" sewer lateral on Grand Ave that will replace the failing system
serving the existing and proposed office use. The location ofthe new sewer lateral and clean-out
shall be approved by the city engineer and shall be processed as a construction revision to
drawing 146-8.
14. Developer shall meet with the fire marshal to determine if fire protection measures (fire flows, fire
hydrant locations, building sprinklers) are required to serve the project. Fire hydrants, if
proposed, shall be considered public improvements and shall be served by public water mains to
the satisfaction of the district engineer.
15. Developer shall pay traffic impact and sewer impact fees based on Section 18.42 and Section
13.10 of the City of Carlsbad Municipal Code, respectively. The Average Daily Trips (ADT) and
floor area contained in the staff report and shown on the site plan are for planning purposes
