Dear Sir or Madam:
Our records indicate that you were issued Permit #97-14 for a/an A-Frame Sign that is
located in the public right-of-way. A requirement of the permit is that the City of
Carlsbad and the Carlsbad Redevelopment Agency be listed as Additional Insured on
your insurance policy. The most recent certificate of insurance on file expired on
12/31/1997. Please ask your insurance provider to ensure that a new certificate is sent to
us immediately and upon renewal annually. If proof of current insurance coverage has
not been received or if the items have not been removed from the public right-of-wav
within 10 business davs of the date of this letter, the A-Frame Sign mav be sunrmiarilv
removed without further notice from the Citv and disposed of as surplus propertv.

Carlsbad Redevelopment Agency, your pennit may be revoked and/or the A-Frame Sign
may be confiscated by Code Enforcement.

Subject: Application for Sidewalk Sign Permit
Dear Mr. Hennessey:
Thank you for submitting your application for a Sidewalk Sign Permit. Your application
has been approved. Attached is a copy of the approved Sidewalk Sign Permit for your
records.
The attached permit allows you to retain your sign for, 1) as long as the Housing and
Redevelopment Commission allows sidewalk signs to remain, 2) as long as the sign
continues to meet the standards outlined vdthin the Carlsbad Village Master Plan and
Design Manual, and 3) as long as the sign is consistent with this permit approval. It is
important to note that the Housing and Redevelopment Commission wdll complete an
annual review of the sidewalk sign regulations to determine their continued applicability.

reviewed your application and the following item needs to be submitted to the
Redevelopment Agency prior to your application being approved:
1. The original copy of the Certificate of Insurance.
