RE: Current Insurance Certificate for an A-Frame sign and Table and Chairs
Dear Mr. Downs:
Our records indicate that you were issued Permits #97-06 and 98-02 for an A-
frame sign and outside table and chairs that are located within the public right-of-
way. An ongoing requirement of the permit is that you maintain a valid <5ertifk:ate
of insurance for the item(s) within the publk; right-of-way naming the City of
Carlsbad and the Carlsbad Redevelopment Agency as additional insured.
This is a courtesy notice to inform you that the certiftcate of insurarKie we
currently have on file is due to expire on September 6, 2002. If you wouW like to
continue to display the A-frame sign and the table and chairs within the public
right-of-way, please have your insurance carrier forward to us an updated
insurance certificate within 10 business days of the expiratk)n date. Otherwfse,
the A-frame sign and the table and chairs must be removed from the public right-
of-way.

Subject: Approval of Sidewalk Sign Permit
Dear Mr. Downs:
Thank you for submitting your application for a Sidewalk Sign Permit. Your appUcation
has been approved. Attached is a copy of the approved Sidewalk Sign Permit for your
records.
The attached permit allows you to retain your sign for, 1) as long as the Housing and
Redevelopment Commission allows sidewalk signs to remain, 2) as long as the sign
continues to meet the standards outlined vdthin the Carlsbad Village Master Plan and
Design Manual, and 3) as long as the sign is consistent with this permit approval. It is
important to note that the Housing and Redevelopment Commission will complete an
annual review of the sidewalk sign regulations to determine their continued applicability.

Dear Sir or Madam:
Our records indicate that you were issued Permit #97-06 for an A-Frame Sign that is
located in the public right-of-way. A requirement of the permit is that the City of
Carlsbad and the Carlsbad Redevelopment Agency be listed as Additional Insured on
your insurance policy. The most recent certificate of insurance on file expires or expired
on 09/05/1997. If the certificate of insurance on file has not yet expired, please ask your
insurance provider to ensure that a new certificate is sent to us upon renewal. If there is
no certificate of insurance on file or if it has expired, please submit a current certificate of
insurance within ten davs (\0). If a current certificate of insurance is not received by the
Carlsbad Redevelopment Agency, your permit may be revoked and/or the A-Frame Sign
may be confiscated by Code Enforcement.

you to retain your sign until January 1, 1995 as long as it continues to meet the sign standards as outlined
within City Ordinance Section 21.41.
