Stewart Jr. 540 Carlsbad Village Dr. Carlsbad, CA 92008 RE: Current Insurance Certificate for an A-Frame Sign Dear Mr. Stewart: Our records indicate that you were issued Permit #97-19 for an A-Frame sign that is located within the public right-of-way. An ongoing requirement of the permit is that you maintain a valid certificate of insurance for the items within the public right-of-way naming the City of Carlsbad and the Carlsbad Redevelopment Agency as additional insured. The certificate of insurance on file expires on June 16, 2002. Therefore, the A-Frame sign must be removed from the public right-of-way once the certificate of insurance expires. If proof of current insurance coverage has not been received or if the items have not been removed from the public right-of-wav within 10 business davs of the date of the certificate of insurance expiration, the items mav be summarilv removed without further notice fi'om the Citv and disposed of as surplus propertv. A requirement of the permit is that the City of Carlsbad and the Carlsbad Redevelopment Agency be listed as Additional Insured on your insurance policy. The most recent certificate of insurance on file expires or expired on 06/01/2002. If the certificate of insurance on file has not yet expired, please ask your insurance provider to ensure that a new certificate is sent to us upon renewal. If there is no certificate of insurance on file or if it has expired, please submit a current certificate of insurance within ten davs (10). If a current certificate of insurance is not received by the Carlsbad Redevelopment Agency, your permit may be revoked and/or the A-Frame Sign may be confiscated by Code Enforcement. Your application has been approved. Attached is a copy of the approved Sidewalk Sign Permit for your records. The attached permit allows you to retain your sign for, 1) as long as the Housing and Redevelopment Commission allows sidewalk signs to remain, 2) as long as the sign continues to meet the standards outlined within the Carlsbad Village Master Plan and Design Manual, and 3) as long as the sign is consistent with this permit approval. It is important to note that the Housing and Redevelopment Commission will complete an annual review of the sidewalk sign regulations to determine their continued applicability. Staff has again reviewed your application and the following information needs to be submitted to the Redevelopment Agency in order for staff to approve your sign: 1. Please provide an original copy of a Certificate of Insurance naming the "City of Carlsbad" and the "Carlsbad Redevelopment Agency." as additionally insured in the amoimt of $1 million Once the above information has been submitted, staff will then approve your sign.
