Certificate Validation Procedure
S. If you are a Woodwork InstittIte LICENSEE:
1 The Administrative Office forwards one original and three copies of this Certificate to the
LICENSEE for their signaturè attesting compliance.
2. The LICENSEE signs all, retaining a copy for their records, then forwards the original to
the architect, a copy to the general contractor,.. and a copy to the Woodwork Institute
Administrative Office.
If you are a Woodwork Institute INSPECTOR: -
The Administrative Office forwards one original and four copies of this Certificate to the
INSPECTOR of record for his or her signature attesting compliance.
The INSPECTOR signs all, retaining a copy for his or her records, then forwards the origi-
nal to the architect, 'a copy to the woodworker, a copy to the general contractor, and a copy to
the Woodwork Institute Administrative Office.
