CONTRACTOR HRS FOREMAN Kevin WEATHER Clear/650 PERMIT NO. 6607 CODE DESCRIPTION OF WORK PERFORMED On-site @ 09:00 hrs at Mallorca Place in Carlsbad Ca, as requested to perform observation and relative Compaction testing during backfill operations for 18" diameter PVC storm drain r e p l a c e m e n t . T h e 5 4 ' linear feet section was located between the storm drain clean-out at station 1 1 + 2 7 t o t h e p o i n t o f connection at station 11+81. . . . . . Upon arrival, contractor was observed placing 3/4" crushed aggregate in the pipe zone to ± 12" over the top of pipe. Excavated material from stockpile was placed as backfill. Material was placed in ±1 2 " l o o s e l i f t s a n d t h e compaction efforts were applied utilizing a compaction wheel attachment mounted in t h e 3 0 5 C e x c a v a t o r . Contractor was observed placing metallic warning tape at ± 2' Over the top of pipe. Performed four in-place density tests utilizing a Troxler® 3430 Moisture-Density G a u g e . T e s t s r e s u l t s were in conformance with the project requirements of 90% relative compaction.
