Please be advised that on May 11, 2016, SB&O, Inc. performed a field survey of the building
foundation. In my professional opinion, the foundation forms for the above referenced building
have been completed in substantial conformance with the precise grading plan. This certification
is limited to the horizontal location of the major building lines to determine compliance with the
setback dimensions shown on the precise grading plans and the finish floor elevations.

Project No. CT 15-05; Grading Plan No. 484-5C
Please be advised that on December 4, 2015, SB&O, Inc. performed a field survey of the
building pad subgrade. In my professional Opinion, based on the data collected, preparation of
the pad has been completed to within ± 0.1' of the pad subgrade shown on the Preliminary
Grading Plan. This certification includes the as-built condition of the pad grade, but does not
include approval of placement and compaction of earthwork within the purview of others.
