fuel spout Do not activate the dispenser when draining gasoline from the vapor side of the VST hose 11 The onsite vapor control system shall be maintained in good working conditions and shall be operated at all times when volatile organic fuels (i e gasoline) are transferred or stored 12 There shall be no fugitive liquid leaks associated with the transfer of volatile organic fuels Fugitive liquid leak means any visible liquid leak at a rate in excess of three (3) drops per minute other than spillage or losses, which occur upon disconnecting transfer lines 13 Access, facilities, utilities and any necessary safety equipment for source testing and inspection shall be provided upon request of the District 14 The permittee shall, upon determination of applicability and written notification by the District, comply with all applicable requirements of the Air Toxic "Hot Spots" Information and Assessment Act (California Health and Safety Code Section 44300 Et Seq ) 15 All Phase-11 fuel dispensing nozzles shall be electrically connected to an earth ground Clearly marked and easily accessible electrical clamp attachment points, which are electrically connected to a verified earth ground (<lohm resistance), shall be installed at each Phase II fueling point 16 The Phase I and Phase II vapor recovery systems including all associated underground plumbing, shall be installed, operated, and maintained in accordance with the requirements Application No 987827 -3- specified in the appropriate ARE E O s and Installation, Operational and Maintenance Manual, if applicable, and District Rules and Regulations 17 The Phase I and Phase II vapor recovery equipment and associated components, except for components with an allowable leak rate as specified by the most recent applicable ARB Executive Order and Certification Procedure, shall be maintained free of liquid leaks and shall be vapor tight Components with an allowable leak rate shall operate within such rate 18 The permittee shall perform a visual weekly inspection of all Phase I vapor recovery components The components to be inspected for damage, loose connections, or leaks are the vapor and product dust caps and gaskets, poppeted dry breaks, vapor and product fill adaptors, spill boxes, including the drain valves In addition, the permittee shall verify that there is no standing gasoline or debris in the spill boxes and that the drain valves are seating properly 19 The permittee shall perform a visual daily inspection of all Phase II components at retail gasoline dispensing facilities The components to be inspected for damage, loose connections or leaks are nozzles, including hold-open latches, hoses, swivels, and breakaways In addition, each nozzle shall be inspected for damaged faceplates, vapor boot guards, spouts and each hose shall be inspected for wear, severe kinks, cracks, and splitting If the combined volume of all gasoline grades dispensed at this facility is less than 750,000 gallons in any consecutive twelve (12) month period, holidays and weekends are excluded from the daily inspection requirements 20 The permittee shall perform a visual daily inspection, excluding weekends and holidays, of all Phase II components at non-retail gasoline dispensing facilities The components to be inspected for damage, loose connections, or leaks are nozzles, including hold-open latches, hoses, swivels, and breakaways In addition, each nozzle shall be inspected for damaged faceplates, vapor boot guards spouts, and each hose shall be inspected for wear, severe kmks, cracks, and splitting If the combined volume of all gasoline grades dispensed at this facility is less than 1,000,000 gallons in any consecutive twelve (12) month period, the inspection shall be performed weekly 21 An inspection, maintenance and repair log shall be kept onsite for at least three (3) years and made available to the District upon request Attachment I, "Inspection, Maintenance and Repair Log," is attached as an example and can be used for this purpose The inspection, maintenance and repair log shall record the daily and weekly inspections and itemize at a minimum the date the defect was found or the date the component was identified as not being in good condition, the date the component was removed from service, the date of call for service, the date of repair/replacement, the affiliation and name of the person performing the repair/replacement, the certified technician identification number from the applicable manufacturer, the make and model number of the component, and whether the component was maintained, replaced, or repaired 22 Any Phase I and/or Phase II component, device or system identified and recorded by the permittee as not being in good condition, based on testing and/or visual inspections or not operating properly shall be repaired, replaced, or adjusted within seven (7) calendar days of Application No 987827 -4- detection in a manner that will bring the facility into compliance with the applicable District Rules and Regulations and the most recent applicable CARB Executive Orders Any Phase I and/or Phase II component or system that is determined to have a defect as set forth in the California Code of Regulations, Title 17, Section 94006, shall not be operated and shall be removed from service immediately The defective component or system shall not be operated until the defect has been repaired or the defective component replaced such that the defect no longer exists 23 The permittee and/or designated agent shall perform an annual inspection to ensure compliance with all applicable District Rules and Regulations, and all permit conditions , An inspection checklist shall be kept onsite for at least three (3) years and made available to the District upon request Attachment J "Annual Inspection and Maintenance Checklist" is attached as an example and can be used for this purpose The inspection shall verify that a The District permit and the required signs are current and posted These signs include operating instructions for the gasoline nozzles, a toll-free phone number specified by the air pollution control officer for the public to call to report problems with a nozzle or a vapor recovery system (applicable to retail facilities only), and a warning sign stating that topping off is prohibited and may result in spillage of gasoline b The facility complies with all permit conditions c The Phase I and Phase II vapor recovery systems are properly installed and comply with the most recent applicable CARB certification procedures and CARB Executive Orders d Each gasoline underground storage tank or compartment has a gasoline submerged drop tube with a discharge opening entirely submerged when the liquid level is six (6 0) inches above the bottom of the tank or compartment e The vent pipes are equipped with the required pressure/vacuum valves and each such valve is properly installed f All connections and fittings inside the dispenser are free of liquid leaks g The lengths and installation arrangements of all dispenser hoses are in compliance with the most recent applicable CARB Executive Orders PHASE II EQUIPMENT 24 The VST Balance Phase II EVR System, including Veeder-Root ISO System, shall be installed, operated, and maintained in accordance with the ARB E O VR-204-E, the ARB approved Installation, Operation, and Maintenance (IOM) Manual for VR-204-E, and District Rules and Regulations A copy of this E O , including the IOM manual, shall be maintained at the facility at all times and be available for review by the District upon request Application No 987827 -5- 25 Only components specified in the ARB E O VR-204-E, Exhibit 1, shall be installed at this facility These components shall be clearly identified by a permanent identification, showing the manufacturer's name and model number, unless specifically exempt from this requirement 26 If installed, optional components for the vapor recovery system, in accordance with ARB EO VR-204-E, Exhibit 1, shall be ARB certified and shall be installed, operated and maintained in accordance with the ARB approved Installation, Operation, and Maintenance Manual for VR-204-E 27 The Phase II system authorized herein shall be equipped with the latest version of the ARB certified Veeder-Root ISD software and shall be setup for use with a Balance System and a V-R Vapor Polisher 28 The TLS Console shall be installed in a location that allows the RS232 port to be easily accessible for use at any time No obstructions shall be placed within a radius of one foot (1') from the TLS Console A vacant RS232 port shall always be available and be able to electronically download complete and accurate reports The TLS Console shall also be equipped with an operational printer (including an adequate supply of paper and toner) 29 The TLS Console shall automatically prohibit the dispensing of fuel through shutdown of all the gasoline turbine pumps upon a failure alarm or TLS Console ISD system power loss 30 The facility attendant shall notify (e g , direct contact, voice mail, e-mail, fax) the responsible company official or their designee within two (2) hours of detecting any ISD warning alarm and shall request service as soon as is reasonably possible to correct the problem Only technicians certified by the manufacturer shall be allowed to make vapor recovery or ISD system repairs 31 The permittee and/or designated service technician shall promptly record all information relating to the alarm event, reporting, and associated repairs on Attachment I, "Inspection, Maintenance and Repair Log," or an equivalent form The inspection, maintenance, and repair log shall be kept onsite for at least three (3) years and made available to the District upon request 32 When an ISD alarm occurs and gasoline dispensing is terminated, the permittee shall reset the ISD system to allow vehicle fueling to resume only if a All required repairs have been made and all information associated with the repairs is recorded in Attachment I, the maintenance and repair log, or b The dispenser(s) associated with the problem that triggered the alarm is isolated and not operated until the required repairs are completed, and all information associated with the repairs is recorded The time the components are isolated shall be logged immediately Application No 987827 -6- 33 The ISD audible alarms shall be installed at a location that is most likely to be heard by the station attendant during normal station operation (e g at the cash register) 34 The permittee and/or designated contractor shall not perform a "Clear Test After Repair" (i e clear alarm conditions) upon any alarm unless all repairs necessary to correct the alarm condition have been successfully performed per Attachment L and recorded in the maintenance log Only a manufacturer certified technician shall perform a "Clear Test After Repair" 35 The vapor recovery ISD system shall be operational for a minimum of ninety-five percent (95%) of the time, based on an annual basis or prorated thereof The operational percentage time shall be verified through the daily reports 36 Each dispenser shall have a certified vapor flow meter as shown in Figures 2B-6, 2B-10 and 2B-11 of Exhibit 2, ARE EO VR-204-E 37 Each gasoline dispensing facility shall have a certified vapor pressure sensor installed into one of the dispensers located closest to the tanks, as shown in Figures 2B-7, 2B-10 and 2B- 11 of Exhibit 2, ARB EO VR-204-E In the event that a row of dispensers are equal distance from the tank pad and within 10 feet of each other, the vapor pressure sensor can be installed into any of the dispensers 38 The ISD system shall maintain a complete electronic archive of monthly reports for a period of 12 consecutive months and an archive of daily reports for the last 365 consecutive days 39 Scheduled maintenance, which includes daily and annual inspections and/or tests, shall be performed in accordance with the ARB approved Installation, Operation, and Maintenance Manual for VR-204-E Annual inspections and testing shall be performed by a technician certified by the manufacturer Only technicians certified by the manufacturer can service any problems discovered while conducting the daily or annual inspections and/or testing 40 A maintenance log for the daily and annual scheduled maintenance, including any repairs performed and drive offs, shall be kept onsite for at least three (3) years and made available to the District upon request The maintenance log shall itemize at a minimum the date of each inspection and test, any defect, damage, loose connections, or leaks found during the inspections or tests, any test failure, the make and model number of any component that is replaced, maintained or repaired as a result of these inspections or tests, the date of repair/replacement, and the affiliation and name of the person performing the inspections, tests, and repair/replacement 41 In accordance with the application submitted, the underground vapor recovery and vent lines shall only be replaced within the tank, vent and dispenser sumps 42 All vapor return lines shall have a minimum slope of 1/8 inch per foot from the dispenser riser to the riser of the UST All vent lines shall be sloped a minimum of 1/8 inch per foot from the vent risers to the storage tanks Application No 987827 -7- 43 The dispenser vapor piping of all new dispensers shall have a minimum nominal internal diameter of one inch (1" I D ) 44 Liquid condensate traps are not allowed at this facility 45 Product shall not be dispensed from any fueling point at this facility if a vapor line is disconnected and open to the atmosphere 46 A Vapor Polisher, manufactured by Veeder-Root, model 332761-002, shall be installed at this gasoline dispensing facility 47 Security seal tags shall be installed on the Vapor Polisher The permittee shall inspect the Vapor Polisher for damage or missing security tags on a monthly basis and document the inspection in the maintenance log Exhibits 11 and 12 of ARB E O VR-204-E shall be successfully conducted and new security seal tags shall be installed within thirty (30) days of finding missing or damaged security tags 48 The carbon type in the Vapor Polisher shall be BAX Gl 500, manufactured by MeadWestvaco 49 Unless maintenance or testing is being performed on the Vapor Polisher, the ball valve located at the inlet of the Vapor Polisher shall be locked in the open position as shown in Figure 2B-3, Exhibit 2 of ARB EO VR-204-E The handle of the ball valve shall not be removed 50 Unless maintenance or testing is being performed on the processor, the Carbon Polisher shall be on and in the automatic vapor processor mode at all times, even when the facility is closed for business 51 The Vapor Polisher Pressure Regulated Valve shall operate (i e open or close) in accordance with the percent load and pressure settings located in Table 6 of Section 16 of the ARB approved Installation, Operation, and Maintenance Manual for VR-204-E 52 Only VST, Model VST-EVR-NB or Model VST-EVR-NB-R, nozzles as shown in Figure 2B-1, Exhibit 2 of ARB EO VR-204-E, shall be installed at this facility 53 The maximum length of the curb hose, breakaway, and whip hose combined shall not exceed fifteen feet (15') as measured from the base of the nozzle to the end of the dispenser adapter or dispenser 54 Flow limiters are not allowed to be installed at this facility CONSTRUCTION COMPLETION NOTICE Application No 987827 -8- '55 Upon completion of construction in accordance with this Authority to Construct and prior to commencing operation, the applicant must complete and mail, deliver personally or via facsimile the enclosed Construction Completion Notice to the District 56 The enclosed construction completion notice shall report the name and Manufacturer Certification Number of the contractor who installed the Phase II EVR system Alternatively, a copy of the Manufacturer Certification Card of the contractor who installed the Phase II EVR system shall be attached to the Construction Completion Notice 57 After submitting the Construction Completion Notice, the applicant may commence operation of the equipment Operation must be in compliance with all of the conditions of this Authority to Construct and applicable District Rules and Regulations CONFIRMATION OF TEST DATE ' 58 Within 30 days of completion of construction and at least fifteen (15) calendar days prior to the proposed test date, the applicant shall contact Suha Haddad at (858) 586-2716 to schedule a test date PERFORMANCE TESTING 59 The following performance tests shall be successfully conducted within 60 days after startup of the equipment authorized herein For the purpose of compliance with this condition, all tests shall be conducted after backfilling, paving and installation of all required Phase II components Within 15 calendar days of completion of the following tests, the applicant/testing company shall submit copies of all test data and results to the District's Vapor Recovery Section of the Engineering Division All test results shall be maintained onsite for three (3) years from test date a The integrity of the vapor valve shall be tested to assure that there are no leaks in the nozzle's vapor valve, in accordance with Exhibit 10 of ARE E O VR-203-E or VR- 204-E or ARE Test Procedure TP-201 2B b A pressure decay leak test of the entire vapor control system, including nozzle/check valve and the underground tanks, shall be performed in accordance with the ARB's Test Procedure TP-201 3 and Exhibit 4 of ARE E O VR-203-E or VR-204-E Under no circumstances shall gasoline be poured into spill containers to check for leaks All liquid in the spill containers and sumps shall be removed prior to conducting this test ARB's Test Procedure TP-201 3 shall be conducted and completed between sundown and a half hour after sunrise to minimize interface from solar effects and barometric pressure changes No fuel shall be introduced into the underground tank(s) for at least three (3) hours preceding this test procedure In lieu of TP-201 3 and Exhibit 4 of ARE E O VR-203-E or VR-204-E, the operator/owner may choose to conduct and submit the results of District leak test method TP-96-1 and Exhibit 4 of ARE E O VR- 203-E or VR-204-E This test can be conducted at any time during the day, except when ambient temperatures exceed 100 degrees Fahrenheit, and there is direct sunlight on exposed metal vent pipe(s) and metal manhole cover(s) that are in contact with Application No 987827 -9- * vapor space of the storage tank(s) No fuel shall be introduced into the underground tank(s) for at least eight (8) hours preceding this test procedure Ensure all P/V valves have been removed and vent risers capped prior to conducting the test and re-mstall the valves after this test has been completed If this facility is identified as a BACT site by the permit to operate, a pressure decay leak test must be successfully conducted per District Test Procedure TP-96-1 and Exhibit 4 of ARB E O VR-203-E or VR-204-E and performance of the ARE test procedure TP-2Q1 3 and Exhibit 4 of ARB E O VR- 203-E or VR-2Q4-E is not an acceptable alternative c A tank vapor manifold tie test, to verify the existence of an underground tank interconnect vapor pipe, shall be performed in accordance with ARB's Test Procedure TP-201 3C Note this test shall be performed after the aboveground vent manifold (if installed) is removed and the vent risers capped off d A Veeder-Root Vapor Polisher Operabihty Test shall be performed in accordance with Exhibit 11 of ARB EO VR-203-E and VR-204-E e A Hydrocarbon Emissions Verification Test shall be conducted in accordance with Exhibit 12 of ARB EO VR-203-E and VR-204-E f A dynamic pressure performance test shall be conducted in accordance with ARB's Test Procedure TP-201 4 (Methodology 1) and Exhibit 14 of ARB EO VR-204-E The pressure drop shall be measured from the nozzle to its associated underground storage tank with pressure/vacuum vent valves installed and with the poppeted Phase I vapor connection open This test shall be performed after liquid is introduced down each dispenser riser into the Phase II system The maximum pressure drop shall not exceed 035 inches WC at 60 standard cubic feet per hour and 0 62 inches WC at 80 standard cubic feet per hour g A liquid removal test shall be conducted in accordance with Option 2 of Exhibit 5 of ARB EO VR-203-E or VR-204-E on each hose installed at this facility If umhose dispensers are installed at this facility, then the liquid removal test shall be performed on the grade with the lowest flow rate The minimum liquid removal rate shall be 5 0 milhliters (ml) per gallon dispensed when tested with a gasoline flow rate between six (6 0) and ten (100) gallons per minute h A Vapor Pressure Sensor Verification Test shall be conducted in accordance with Exhibit 8 of ARB EO VR-203-E and VR-204-E i An ISD Flow Meter Operabihty Test shall be performed in accordance with Exhibit 13 of ARB EO VR-204-E j A Veeder-Root Vapor Polisher Valve Position Operabihty Verification shall be conducted in accordance with Attachment M 60 The performance sequence of all the above tests shall be determined by the District prior to the start of testing at this site and shall be strictly followed by the testing company Application No 987827 -10- Performance of these tests simultaneously, proceeding with other tests after failure of a previous test or re-testing at this site shall only occur upon District approval If the tests listed above reveal system problems or inadequacies, corrective repairs shall be made Pre- testing of the vapor recovery system is an option for the applicant This is done to identify and correct problems before the District witnesses official testing Pre-testing can eliminate the time and expense of having to perform a second test and pursue a variance until a second test has taken place.
