Dear Mr. Radovanovic: The following fmdings and observations were noted on the above referenced project: Status of Construction: The contractor has completed 100%of the foundation and slab on grade reinforcing work and it is getting ready to pour the footings. Site Observations and Recommendations: 1.The contractor shall remove any sand, loose soil, or other debris from the bottom of the footings before the concrete pour. 2.Contractor to verify all anchor bolt locations and projections from the top ofthe concrete footings in order to get the proper nut placement at all stud walls. 3.The contractor shall verify all reinforcing bar clearances before placing the concrete. 4.Provide proper protection from future traffic for the slab dowels without damaging them. 5.The contractor shall ensure that all reinforcement, anchor bolts; holdown bolts, etc. is securely tied in place prior to concrete placement. 6.At Grid Lines "A",between "2.3 and "2.6'"', at pipe penertration through the concrete stem wall above the footing, contractor to provide additional 2-#4 x 4'-0" long dowels above the pipe, about 2" clear from the top ofpipe. 1 ,, 7.The contractor shall notify FOR for an additional site observation when the roof plywood sheathing is substantially complete. Initially this questionnaire must be completed by the applicant in advance of submitting for a development application (subdivision, discretionary permits and/or construction permits).The results of the questionnaire determine the level of storm water standards that must be applied to a proposed development or redevelopment project. Depending on the outcome, your project will either be subject to 'Standard Stormwater Requirements'or be subject to additional criteria called 'Priority Development Project Requirements'.Many aspects of project site design are dependent upon the storm water standards applied to a project. As another example say the project included a stockpile that you intend to cover with a plastic sheet. Since plastic sheeting is not on the list of BMPs, then write in "Cover with Plastic"in the blank column under the heading Erosion Control BMPs. Then place an X in the box where the "Stockpiling" row intersects the new "Cover with Plastic" column.
