In our Listing and Follow-Up Service, we are dedi-
cated to offering product evaluation, quality control
inspections and certification services-giving you
the best possible quality at the best price.
Our staff has extensive experience in the areas of
testing, engineering, code requirements/compli-
ance, and inspection techniques.

the National Evaluation Service, Inc. (NES), ;IS a
Quality Assurance/Inspection Agency and has been
issued a National Evaluation Report
(NER-QA33 7).

Establishing the Listing and FolIoWUp Service
This is the procedure we follow in setting up a
product in the Omega Point Listing and Follow-Up
Service. It allows for prompt entry into the pro-
gram with minimal delays and expense.
1. The applicant supplies information on all
manufacturing locations, the intended end-
use of the product, and any pertinent
installation details.
2. Omega Point prepares a written proposal
containing the details of any testing, ser-
vices supplied by Omega Point, and time
and fee schedules.
3. The applicant returns to Omega Point a
signed copy of the Application for testing
and/or Listing and Follow-Up Service along
with any initial payment, as requested.
4. The production of the test specimens and
the initial QC Inspection is scheduled.
5. The applicant submits a completed Quality
Control Manual Information Form. All
in format ion concern i ng product for mi u la -
tion or any proprietary data is considered
strictly confidential.
6. Omega Point witnesses production and
selects test specimens.
7. Appropriate tests are scheduled and run at
our laboratory or, on rare occasions, at
other qualified laboratories. The applicant’s
representatives may be present for the
product testing.
8. When tests requirements are met, Omega
Point prepares a Listing and Follow-Up
Service Agreement that is signed by both
parties.
The Quality Control Manual
The Quality Control Manual serves as a complete
description of the manufacturing process, a quality
control manual for the Omega Point field inspector
to follow during plant audits, and as a guide to the
reporting of audit results.
Manufacturing Process
This manual contains a detailed description
of the entire manufacturing process includ-
ing all checks on raw materials product speci-
fications, quality control procedures during
manufacture, final inspection, and packaging
for shipment.
Discrepancies
The manual contains precise instructions for
handling discovered discrepancies, removal
and disposal of the Omega Point label from
unqualified products, and submittals of writ-
ten guarantees that the problems have been
resolved before labeling is allowed to resume.

Minor Discrepancies
Any minor discrepancies which do not nvcts-
Sitdte cessation of production or labeling
privileges are written on the Quality Control
Audit Report form. The manufacturer has 10
days to rectify the problem and notify Omega
Point of the corrective action.
Severe Discrepancies
Severe discrepancies result in immediate noti-
fication to Omega Point and the plant man-
ager or designee. All labeling privileges are
suspended until the discrepancy is resolved
to the satisfaction of both parties. In some
cases, retesting may be required to requalify
the product or to demonstrate that the dis-
crepancy was not detrimental to the perfor-
mance of the product.
Quality Control Audit
The Inspection Procedure Manual allows Omega
Point field engineers to conduct an unannounced
quality control audit. The audit looks for changes
in the manufacturing methods, and degradation or
changes in raw materials, components, and fin-
ished products.
After the quality control audit, the field inspector
completes the Audit Report forms supplied with
the manual. These forms must be signed by the
plant manager or the appropriate designee. A copy
is left with the signer; the orginal form is returned
to Omega Point.
Manual locations
A copy of the manual is kept on location by
the plant manager or designee and a copy
accompanies the Omega Point field inspector
during the Quality Control audit. .An addi-
tional copy IS maintained at omega Point's
office in San Antonio. reuas
Manual Changes
Only Omega Point personnel are allowed to
make changes, deletions or additions to the
Quality Control Manual. To update the rnan-
uals, Omega Point will recall all field copies;
revise the manual, incorporating the
changes; re-issue the manuals to the field
inspector and the manufacturer; and include
a letter of explanation pointing out all
changes with an explanation for the changes.
For security purposes, the Quality Control
Manual is treated as a controlled document.
Change In Materials and/or Manufacturing Site(s)
While a product is. listed by Omega Point, the man-
ufacturer must notify Omega Point, in writing, of
any changes in suppliers of raw material, compo-
nents, or anything else which may cause a change
in the finished product.
Any changes in manufacturing techniques deemed
significant by Omega Point engineers will result in
retesting of the product as per the original condi-
tions under which the product was qualified.
The manufacturer is also required to notify Omega
Point of any changes in manufacturing site(s) as
they are established.
Listing and FollowUp Service Agreement
The manufacturer executes the Listing and Follow-
Up Service Agreement before any product is listed
and certified by Omega Point.
After the Agreement is executed:
1. Authorization is given to affix the Omega
Point Listing Mark to each specimen.

products to the published listing.

formed as specified in the manufacturer's
Quality Control Manual.
4. Periodically, Omega Point inspectors select
and mark product specimens for shipment

The Omega Point label provides evidence and doc-
umentation to support the product's ability to meet
specified requirements.
Omega Point's registered mark- shown on the
right-is the mark, seal, or label that a manufactur-
er is authorized to attach or imprint on the product
after all applicable test requirements have been met
and approval has been granted. -
Description of Mark
Within the mark is an identification number that is
traceable to the manufacturer, the product, and the
manufacturing site. Also within the mark is an
identification of the test method and test results of
the certified product.
When sequential serial numbers are used, they are
included with the mark.

INTRODUCTION
The 1994 Uniform Building Code (UBC 15-2) provides a basis for relative
comparison of fire performance of roof coverings. The test methods include simulated fire
exposure to the outside of the roof coverings, and, where applicable, a determination as to
whether the fire performance characteristics of the roof covering may be adversely affected
by prolonged exposure to rain and weathering. They are applicable to roof coverings
intended for installation on either combustible or non-combustible decks, when applied as
intended for use.
The following test methods are included:
Intermittent Flame Test .
Spread of Flame Test
Burning Brand Test
All or some of the above tests are used to classify the roof coverings mainly
depending on whether the roof covering is or is not restricted for use on noncombustible
decks. Three classes of frre test exposures are described:
Class A Tests are applicable to roof coverings that are effective against severe test
exposure, afford a high degree of fire protection to the roof deck, do not slip from position,
and do not present a flying brand hazard.
Class B Tests are applicable to roof coverings that are effective against moderate test
exposure, afford a moderate degree of fire protection to the roof deck, do not slip from
position, and do not present a flying brand hazard.
Class C Tests are applicable to roof coverings that are effective against light test
exposure, afford a light degree of fire protection to the roof deck, do not slip from position,
and do not present a flying brand hazard.

TESTING CON DUCTED SUMMARY
A series of UBC 15-2 tests were conducted for the client Slatelselect, Inc., 3162
Miller Park Dr. North, Garland, TX 75042-7759 on March 5.6.7 and April 18, 1996.
The sample materials were received on January 29, and April 15, 1996.
Two Class A Intermittent Flame Tests, two Class A Spread of Flame Tests, and
four Class A Burning Brand Tests (a total of eight tests) were conducted on fiberglass
reinforced cementitious slate decks of the same composition.

TESTING CO NDUCTED SUMMARY
A series of UBC 15-2 Class B fire tests were conducted for the client:
SlatdSelect, Inc.

Garland, TX 75042-7759
A total of four tests were conducted on samples that were comprised of fiberglass
reinforced cementitious slate on 2 layers of 3W 36” wide rolled roofing felt underlayment
and 7/16” ACX plywood sheathing.
Two Class B Burning Brand Fire Tests were conducted on May 1, 1996.
Two Class B Intermittent Flame Fire Tests were conducted on June 6,
1996.

CONCLUSIONS
The 4 decks submitted for testing by Slate Select, Inc. and described in this report
did meet the UBC 15-2 acceptance criteria when subjected to the Class B Burning
Brand Fire Test and Class B Intermittent Flame Fire Test.
