A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF
2 CARLSBAD, CALIFORNIA, AUTHORIZING THE EXPENDITURE OF
3 FUNDS FOR SETTLEMENT OF THE WORKERS' COMPENSATION
CLAIM OF DAVID MTTCHLEY.

5 WHEREAS, TRISTAR, the City's Third Party Administrator, has recommended a

settlement of the Workers' Compensation claim of David Mitchley; and

WHEREAS, the settlement has been approved by the Workers' Compensation
8
Appeals Board; and
I g WHEREAS, there are sufficient funds available in the Workers' Compensation
I1 Self-Insurance Fund to pay the settlement.
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Carlsbad,

California, as follows:

1. That the above recitations are true and correct.

2. That the expenditure of $73,755.00 from the Workers' Compensation Self-
•) 7 Insurance Fund is authorized for the settlement of said claim.
18 3. That the City Council accepts the Stipulations with Award on file in the
19 Human Resources Department.

of the City of Carlsbad on the 11 th day of July, 2006, by the following vote:
