Re: Street Cut Permit
During our regular RF leakage patrol, we discovered a major leak radiating from our
underground plant at 3070 Blenkarne Drive in Carlsbad. This street is an older street with
direct-buried cable, thus requiring a complete replacement of the span between the nearest
pedestals. We issued a work order to The Fishel Company to replace this span. We were
issued a Right of Way Permit from the City of Carlsbad on 1/27/2000 (Permit No. RW000017
attached Of course, these permits are nor actually valid until the City Inspector signs off at
thejobsite).
A Fishel representative, Ron Hanks, met on site with the City Inspector, Joe McMahon. Joe
would not approve the permit on the basis that the street had been overlaid within the past 3-
years. He indicated to Ron that there was some decision pending on the moratorium issue, but
did not commit to any particular follow up action. At this point, we have a 200 micro-
volt/meter leak, which is considered a major leak by the FCC. By law we must repair this leak
immediately, however we are unable to effect this repair.
Ron tells me that there are no economically viable options to cutting the asphalt due to the
terrain, sidewalk location and retaining walls. It is important to note that this street is also
slated for a complete rebuild in the coining months.

INSPECTION/INSPECTOR AVAILABILITY
PRE-CONSTRUCTION MEETING: Within 48 (business) hours of your permit being
approved, an Engineering Inspector will notify permittee and arrange a p re-
construction meeting. The permit will be issued at the end of the pre-
construction meeting.
After the pre-construction meeting, all inspection requests will be made by calling the
engineering 24-hour inspection request line at (760) 438-3891. Inspection requests
must be received prior to 2:00 pm on the day before the inspection is needed.
Inspections will be made the next work day following the request unless you request
otherwise. Requests made after 2:00 pm will be scheduled for two business days later.
FAILURE TO GIVE THE FOLLOWING INFORMATION WILL RESULT IN A
CANCELLATION OF YOUR INSPECTION REQUEST:
