The Contractor is directed to make ·the following changes as described herein.
Changes shall include all labor, materials, equipment, contract time
extensions, and al other goods and services required to implement this
change. Payment is stated on this change order includes all charges, direct
or indirect, arising out of this additional work and is expressly agreed
between Agency and Contractor to be the complete and final costs hereof. The
requirements of the specifications, where pertinent and not in conflict with
this change order, shall apply to these changes. This change order is not
effective unless signed by the Commission Secretary or its Chairman.
CHANGE:
Install 104 square feet of brick pavers on utility box lids at $29.0 per square foot.
REASON FOR CHANGE:
Unsightly tra~fic signal and street light fuse boxes may ... e hidden beneath the sidewalk
brick pavers.

CHANGE:
Install 4'X4'X3' deep octagonal foot1ng with 4--0.625" dia. anchor bolts to support
k1osk structure in sidewalk.
REASON FOR CHANGE:
f This allows installation of a new Streetscape-style directional sign in the Elm Avenue
sidewalk.
