Test Hole Data Sheets PREPARED BY AIRX UTILITY SURVEYORS, INC. 2534 EAST EL NORTE PARKWAY, UNIT C ESCONDIDO, CA 92027-3751 www.airxus.com INTRODUCTION AIRX Utility Surveyors performs a full range of specialized engineering services including Underground Utility Location and Vacuum Excavation (to verify depth and alignment of underground utilities). Underground Utility Location employs sophisticated electronic locating devices to trace the route of an underground utility; then the route is marked on the surface with marking paint in paved areas, or with 60 penny nails and feathers in unpaved areas. Vacuum excavation employs a vacuum truck in conjunction with high-pressure air or water to excavate material and expose an underground utility. A 12 to 18 inch diameter hole is first cored or chiseled through the pavement so that the vacuum hose and high pressure air or water hose can be inserted. Utilities found during potholing are located on the surface by placing two reference points (typically 36" apart) on either side of the pothole along the centerline (or edge) of the utility, using paving nails or 60 penny nails as appropriate. Depth measurements (to the nearest Vi inch) are taken midway between the two reference points. Utility alignment, depth, and other data is marked with pink paint (on pavement), or on wood lath (unpaved areas). PROJECT DESCRIPTION AND COMMENTS AirX was retained by Sherri Howard, Associate Engineer ofthe City of Carlsbad to verify depth and location of utilities on Grand Avenue in Carlsbad, CA. The information obtained will assist design work forthe proposed NW Quadrant Storm Drain Project (No.6608). In preparation of these excavations, AirX marked out the dig locations and called in for USA (Underground Service Alert). In order to verify the alignment, which had changed, Steve Tueting with AirX did two job walks with Sherri Howard from the City of Carlsbad. In the report everything that runs in line with Grand Avenue is being labeled as E/W, and everything that directly crosses Grand Avenue is labeled as N/S. The first job walk was to verify pothole locations, as several potholes were clearly a distance from the proposed alignment. The second was to critique the pothole locations after the alignment had been shifted and the designer from RBF was present to answer questions and note conflicts. Traffic control plans and permits were drawn and applied for by AirX. No parks were set as required by the city. The original start date of the project was set back a week due to a Cinco de Mayo event in downtown Carlsbad,rainy weather also contributed to the delayed start time. There were abandoned fire hydrant service laterals and sections of 10" ACP water pipe along Grand Avenue. The AirX crews spent four days with the use of hydro-excavation equipment potholing for utilities. Any utilities found in the potholes that were in addition to the utilities originally requested have been photographed and documented and are included on the data sheets. At the request of the City Inspector, AirX crews pressure washed the USA mark out off of the sidewalks and utilized a coring machine to cleanly cut through the decorative brick in the intersection of State Street and Grand Avenue. AirX used potable water obtained offsite for excavation, and all water and material vacuumed up was disposed of offsite; no water was allowed to fiow into storm drains or natural drainage. The potholes were covered with clean backfill material and compacted to 95% density in 3" lifts. The pavement was patched with an equal thickness of replacement asphalt. Underground Utility Location employs sophisticated electronic locating devices to trace the route of an underground utility; then the route is marked on the surface with marking paint in paved areas, or with 60 penny nails and feathers in unpaved areas. Vacuum excavation employs a vacuum truck in conjunction with high-pressure air or water to excavate material and expose an underground utility. A 12 to 14 inch diameter hole is first cored or chiseled through the pavement so that the vacuum hose and high pressure air or water hose can be inserted. Utilities found during potholing are located on the surface by placing two reference points (typically 36" apart) on either side of the pothole along the centerline (or edge) of the utility, using paving nails or 60 penny nails as appropriate. Depth measurements (to the nearest inch) are taken midway between the two reference points. Utility alignment, depth, and other data are marked with pink paint on the pavement in the pothole area. PROJECT DESCRIPTION AND COMMENTS AirX was originally retained in May 2013 by Sherri Howard, Associate Engineer of the City of Carlsbad to verify depth and location of utilities on both Highland Drive, Basswood Avenue and Linda Lane in Carisbad, CA. The information obtained was to assist design work for the proposed Northwest Quadrant Storm Drain Improvement Project (No.6608). Between June 21-27 2013 AirX excavated 20 potholes on Linda Lane and 8 potholes on Highland Drive for project total of 28 potholes. All data collected was Submitted in report format on July l"2013. AirX was contacted by Sherri Howard in late July 2013 to provide additional utility locating and potholing services for the Northwest Quadrant Storm Drain Improvement Project. In preparation of these excavations, AirX marked out the dig locations and called in for USA (Underground Service Alert). In order to verify the alignment, Steve Tueting of AirX completed a pre-dig walkthrough with Sheri Howard. Traffic control plans and permits were drawn and applied for by AirX. No parks were set as required by the city. All potholing was performed between August 14-19 2013 using hydro-excavation equipment. Any utilities found in the potholes that were in addition to the utilities originally requested have been photographed and documented and are included on the data sheets. A total 17 potholes were excavated during this project on three different streets in city of Carlsbad; Linda Lane (5 potholes; PH 101-103,105-106), Grand Avenue (8 potholes; PH 201-208) and Basswood Avenue (4 potholes; PH B1-B3, B5). Specific observation include the moving of potholes 201 and 203 on Grand Avenue approximately one foot SW from the original locations in order to preserve existing decorative pavers and still locate utilities within 0 Page 2 of3 o the pothole area. Unfortunately the utility layout for pothole 202 did not allow the pothole location to be moved off of decorative pavers. On Basswood Avenue the alignment of the pipe was unclear so a plan was crafted to emulate slot trenching to find it initially at one location. Pothole B3 was to be excavated first and if unsuccessful B4 wouid be opened. At B3 the pipe was found and therefore B4 was not used. The pipe was found at the other locations Bl, B2 and B5 although the pipe alignment is not parallel with the curb. AirX used potable water obtained offsite for excavation, and all water and material vacuumed up was disposed of offsite; no water was allowed to flow into storm drains or natural drainage. The potholes were covered with clean backfill material and compacted to 95% density in 3" lifts. Underground Utility Location employs sophisticated electronic locating devices to trace the route of an underground utility; then the route is marked on the surface with marking paint in paved areas, or with 60 penny nails and feathers in unpaved areas. Vacuum excavation employs a vacuum truck in conjunction with high-pressure air or water to excavate material and expose an underground utility. A 12 to 14 inch diameter hole is first cored or chiseled through the pavement so that the vacuum hose and high pressure air or water hose can be inserted. Utilities found during potholing are located on the surface by placing two reference points (typically 36" apart) on either side ofthe pothole along the centerline (or edge) of the utility, using paving nails or 60 penny nails as appropriate. Depth measurements (to the nearest inch) are taken midway between the two reference points. Utility alignment, depth, and other data are marked with pink paint on the pavement in the pothole area. PROJECT DESCRIPTION AND COMMENTS AirX was originally retained in May 2013 by Sherri Howard, Associate Engineer ofthe City of Carlsbad to verify depth and location of utilities on Grand Avenue between the North County Transit Railroad crossing and Madison Street in Carlsbad, CA. The information obtained was to assist in design work for the proposed Northwest Quadrant Storm Drain Improvement Project (No.6608). Between October 24''*' to 31" 2013 AirX excavated 7 potholes on Grand Avenue in the street (near the tracks) and planter areas along the south side of Grand Avenue. All data collected was Submitted in report format on November f * 2013. In preparation of these excavations, AirX marked out the dig locations and called in for USA (Underground Service Alert). In order to verify the alignment, Steve Tueting of AirX completed a pre-dig walk-through with Sheri Howard. Traffic control plans and permits were drawn and applied for by AirX. No parks were set as required by the city. Traffic control was set up by AirX and potholing was allowed by the city on all days except Wednesday when the Farmer's Market is in operation between Roosevelt and Madison Streets. All of the excavations were successful with the exception of potholes PH403 and PH408 which were not excavated. All of the pertinent data was marked on the street and has been incorporated in this report. AirX used potable water obtained offsite for excavation, and all water and material vacuumed up was disposed of offsite; no water was allowed to flow into storm drains or natural drainage. The potholes were covered with clean backfill material and compacted to 95% density in 3" lifts. The pavement was patched with an equal thickness of replacement asphalt. SITE SPECIFIC DETAILS AIRX was contracted by Sherri Howard from City Of Carlsbad - Public Works Department of Carlsbad, Califomia to perform utility locating and potholing services at Grand Avenue in the City of Carlsbad, Califomia. A total of 15 potholes were excavated to establish conflicts and coimection points on a variety of utilities. The results of the data are contained in the attached spreadsheet and data sheets. In preparation of these excavations, AIRX marked out the dig locations and called in for USA. (Underground Service Alert). Existing traffic control plans and permits were reactivated and applied for by AIEOC. The traffic control equipment was set up by AIRX. All ofthe excavations and discovery went without any major incident except for potholes 516 and 517 which were canceled per Scott Cartwright. In addition water utility in pothole 504 was not found as it mns in N/S direction at the pothole 505 and doesn't mn through pothole 504. In pothole 513 water and sewer utilities were not found as the pothole crew reached big chunks of concrete at the depth of 91". Sewer utility marked out in pothole 513 was dipped in nearby manholes at the depth of 151 inches T/P and 149 inches T/P. The 12" water was potholed about 8 feet to the north and south with consistent depth. All changes and explanations from the original pothole plan are mentioned in the Pothole Tabulation spreadsheet and Record Of Test Hole Data sheets. All the results were annotated on the ground and in this report. It is AIRX's recommendation that all pothole locations and the active mark out should be surveyed to aid in the adjustment utilities on the final plan and profile. At each pothole location, the utility alignment was marked with pink paint, as well as 60 penny nails or feathers. APPENDIX K NCTD DRAFT WORK PLAN NCTD Work Plan Grand Avenue Storm Drain Improvements November 11, 2014 Project Purpose The purpose ofthe overall proposed project is to alleviate drainage issues along Grand Avenue in the City of Carlsbad. This will be accomplished by replacing the existing storm drain system with lager pipes and also replacing several existing inlets. A small portion ofthe project would encroach into the North County Transit District (NCTD) right-of-way (ROW). The purpose of the work within the NCTD ROW is to simplify maintenance by replacing an existing grate inlet with a regional standard curb inlet. The location of the project is mile post 229.20 Project Scope The project extends along Grand from Jefferson Street to the State Street Alley, which is the northeast corner ofthe Carlsbad Visitor Information Center parking lot. Again, only a small portion ofthe overall project is proposed within the NCTD ROW (see Figure 1 for a vicinity map). The scope ofthe work within the NCTD ROW involves the replacement of an existing grate inlet with a new regional standard curb inlet. Minor curb and gutter improvements are also proposed to tie the new inlet into the existing surface improvements. New landscaping will also be provided to match the adjacent planter. All other work associated with the overall project will be performed outside of the NCTD ROW. The existing grated storm drain inlet will be replaced with a 9-foot, Type B-l curb inlet per SDRSD D-02, as detailed in Appendix I. iNLET LOCATION Figure 1 - Vicinity Map/Project Location NCTD - Grand Avenue Inlet Replacement Work Plan November 11, 2014 Page 1 Means and Methods The existing inlet will be demolished and removed. The new inlet will be constructed in the same location and the existing pipes will be reconnected to the new inlet. Minor curb, gutter, and AC improvements will provide a smooth transition from the new inlet to the existing surface improvements. Equipment The contractor will utilize standard construction equipment associated with demo and removal ofthe inlet. Contractor will select the methods and means to perform the removal ofthe existing grated inlet and the construction ofthe standard curb inlet. No equipment will be stored within the NCTD FtOW. Boring/Pothole Locations and Depths No boring or potholing will occur within the NCTD ROW. Schedule Although the overall project will take place over a longer timeframe, the work within the NCTD ROW is expected to begin April 1, 2015 and end April 3, 2015. Noise, Dust or Other Impacts from Work/Equipment The proposed work within the NCTD ROW will have minimal impacts. All construction activities will be performed in accordance with City regulations to best manage noise, dust, and other impacts and per the contract documents. The proposed improvements will result in improved drainage conveyance within the NCTD ROW since the new inlet has a higher capture capacity and is less likely to clog. This project is exempt for resource agency permits. The project must comply with Carlsbad Municipal Code Chapter 8.48-Noise, which regulates permitted hours for construction. All internal combustion engines used in the construction shall be equipped with mufflers in good repair when in use on the project with special attention to the City Noise Control Ordinance, Carlsbad Municipal Code Chapter 8.48. Throughout all phases of construction, including suspension of work, and until the final acceptance, the Contractor shall keep the site clean and free from rubbish and debris. The Contractor shall also abate dust nuisance by cleaning, sweeping and sprinkling with water, or other means as necessary. The use of water resulting in mud on public streets or right of way will not be permitted as a substitute for sweeping or other methods. When required by the Plans or Specifications, the Contractor shall furnish and operate a self-loading motor sweeper with spray nozzles at least once each working day for the purpose of keeping paved areas acceptably clean wherever construction, including restoration, is incomplete. Materials and equipment shall be removed from the site as soon as they are no longer necessary. Before the final inspection, the site shall be cleared of equipment, unused materials, and rubbish so as to present a satisfactory clean and neat appearance. Sufficient material may remain for use as backfill if permitted by the Specifications. Forms and form lumber shall be removed from the site as soon as practicable after stripping. Failure ofthe Contractor to comply with the Engineer's cleanup orders may result in an order to suspend work until the condition is corrected. No additional compensation will be allowed as a result of such suspension. Cleanup and dust control required herein shall also be executed on weekends and other non-working days when needed to preserve the health safety or welfare ofthe public. The Contractor shall conduct effective cleanup and dust control throughout the duration ofthe Contract. The Engineer may require increased levels of cleanup and dust control that, in his/her sole discretion, are necessary to preserve the health, safety and welfare ofthe public. Cleanup and dust control shall be considered incidental to the items of work that they are associated with and no additional payment will be made therefore. The Contractor shall not discharge smoke, dust, or any other air contaminants into the atmosphere in such quantity as will violate the regulations of any legally constituted authority. Traffic Control Formal traffic control plans for the overall project (including the work within the NCTD ROW) will be developed priorto construction and submitted to the City and NCTD for review and approval. Community Outreach Program Community outreach for the overall project has been ongoing. Business owners have been made aware of the planned improvements. Additional outreach will be provided as the start of construction approaches. How Right of Way Will Be Accessed Access to the storm drain inlet is not inhibited by fencing or railway hazards. Access will be from Grand Avenue. Hazards, Spills and Safety Hazards, spills and safety concerns will be addressed as part of the construction of the overall Grand Avenue storm drain system upgrade and in accordance with City, State, and Federal regulation. Prior to construction, the contractor will submit a spill response plan for City review and approval per the contract documents. Materials that contain hazardous substances or mixtures may be required on the Work. A Material Safety Data Sheet as described in Section 5194 ofthe California Code of Regulations shall be requested by the Contractor from the manufacturer of any hazardous products used. NCTD - Grand Avenue Inlet Replacement Work Plan Page 3 November 11, 2014 Material usage shall be accomplished with strict adherence to California Division of Industrial Safety C requirements and all manufacturer warnings and application instructions listed on the Material Safety Data Sheet and on the product container label. The Contractor shall notify the Engineer if a specified product cannot be used under safe conditions. The proposed improvements will improve the long term safety in the area as it relates to drainage. Less frequent and less extensive ponding of stormwater runoff is expected. Emergencies Prior to construction of the overall project, the contract will coordinate with the City to ensure that emergency contacts are identified. In case of emergency, the contractor should call 9-1-1 and NCTD 24-hour security (760) 966-7600.
