Item 2: Refer to Plan Set 1 of 2, Sheets 7 and 8, Stations 169+00 to 172+00. Additional reference is made to City of Carlsbad's transmittal to Steve Bubalo Construction
Company dated 12/14/01. Contractor shall furnish and install a 9000 gallon "Baker"
tank, isolate and stockpile potentially contaminated tunneling spoil, conform to the
City's Community Health and Safety Plan and implement necessary revisions to the
Contractor's Employee Protection Plan. The City will furnish additional direction to the
Contractor regarding the means and methods for disposal of contaminated solid and
liquid wastes. The Contractor shall dispose of contaminated waste as directed by the
City as a provision of this Change Order Item.

The Contractor shall expose a conflicting Pacific Bell telephone duct from Station 145+20 through 145+80 and prepare sub-grade for relocation by Pacific Bell forces. All work shall be performed as directed by the City's Field Representatives. Removal and Replacement of Curb, Gutter and Sidewalk shall be performed in accordance with Unit Prices established within the Contract Documents.

Item 2: Refer to Plan Set 1 of 2, Sheet 16. The Contractor shall remove approximately 75 lineal feet of 10" ACP waterline at approximate Station 148+40 and replace with 10" C-900 PVC waterline. The Contractor shall also remove two (2) 1 O" x 1 O" tee outlets
and associated valves and replace with one (1) four-way 10" cross and valve
assembly. The City shall furnish the new four-way 1 O" cross and valve materials preassembled and ready for installation by the Contractor. All work shall be performed at
the direction of the City's Field Representatives.

In accordance with Section 3-4, Changed Conditions, of the Standard Specifications forPublic Works Construction (2000 as amended), this fax transmittal shall serve as written notification that we have discovered a subsurface condition that differs from the Contact Plans.
During trench excavation for the proposed 12-inch PVC sewer line along Chestnut Ave. on May 1, 2002, SBCC uncovered a concrete thnJst block at approximate Sta. 14+15 that is in direct conflict with the proposed sewer and Sewer Manhole No. 108 at Sta. 14+11. The thrust block is approximately 5-feet from street grade.
