This memorandum provides follow-up information to the previous memorandum submitted to
the Carlsbad Municipal Water District Board of Directors dated March 3, 2022, as shown in
Attachment A, related to the Feb. 24, 2022, response to Hope Elementary School after a Carlsbad
Unified School District, or CUSD, contractor, Erickson-Hall Construction Company, or Erickson-
Hall, reported a water quality issue at Hope Elementary School.
Background
On Thursday, Feb. 24, 2022, Carlsbad Municipal Water District, or CMWD, staff received a call
from the Project Manager for Erickson-Hall Construction Company, the CUSD's contractor,
requesting assistance with a water quality issue at Hope Elementary School. The Erickson-Hall
Project Manager indicated that recent samples that were taken at Hope Elementary School
showed higher levels of copper than would normally be found in potable water. CMWD staff
immediately responded to Hope Elementary School and began investigating the situation.
Discussion
Since the last memorandum to the Board, Attachment A, CMWD staff completed the
investigation and submitted an Incident Report to the State Water Resources Control Board, or
the State Water Board, on March 16, 2022, Attachment B. While the prior memorandum stated a
final report due date of March 4, 2022, the State Water Board granted the CMWD an extension
to March 16, 2022, and the Incident Report was submitted in a timely manner by the
March 16, 2022, deadline.
This Incident Report explains each step that was taken to correct the immediate water quality -
situation at Hope Elementary School, remove the public supply of recycled water from the site,
restore the public potable water supply, and allow Hope Elementary School to resume operation
on Monday, Feb. 28, 2022. It also includes a description of the cross-connection that CMWD staff
found behind a relocatable classroom on the west side of the campus, where Erickson-Hall is
under active construction. The report also notes that the CUSD continued construction work
after the city issued a Stop Work Order to the CUSD on July 30, 2020.
The CMWD's recycled water rules and regulations require that if any changes are made to a
recycled water irrigation system on private property, the owner making changes must submit an

Irrigation Plan for review and approval by the CMWD and the San Diego County Department of
Environmental Health and Quality, or DEHQ. Neither the CUSD nor Erickson-Hall had submitted
any plans to the CMWD or the County DEHQ for any work performed on Hope Elementary
School's recycled water system. Despite this requirement, recycled lines were moved during
construction. Recently, after the incident occurred, the CUSD contractor provided a Utilities Plan
to the CMWD that had been approved by the California Division of the State Architect. The west
area of the construction site, which is where the cross-connection occurred, was not included on
the plans and no changes to the recycled water system were shown.
During the CMWD's investigation of this matter, staff discovered that Hope Elementary School
had been receiving comments about potable water quality on campus since August 2021.
Although staff does not know the exact date when the cross-connection occurred, it appears that
it happened around August 2021.
The recycled water system at Hope Elementary School continues to be disconnected. The CUSD
contractor continues to work through the process that the CMWD and the DEHQ will require
before Hope Elementary School will be allowed to reconnect to the recycled water system for
irrigation.
Next Steps
The CMWD will continue to work with the State Water Board and the DEHQ to reinforce the
CMWD's authority over recycled water regulations at school sites. In addition, CMWD staff will
work with the CUSD and the DEHQ to schedule cross-connection testing of the other Carlsbad
schools that have recycled water and projects under construction. This will determine whether
any changes have been made to those water systems and whether the potable and recycled
water being served at those other schools meet all applicable regulatory standards.

This memorandum provides information related to the Feb. 24, 2022, discovery of a cross
connection between the recycled and potable water lines at Hope Elementary School, located at
3010 Tamarack Avenue. A cross-connection is a point in a water system where the potable, or
drinking, water supply is connected to a non-potable source, in this case recycled water. The
cross-connection involved Carlsbad Unified School District, or CUSD, owned pipes only and
appears to be associated with the CUSD's ongoing construction project at the school.
Background
On Thursday, Feb. 24, 2022, the CMWD received a call from the Project Manager for Erickson
Hall Construction Company, CUSD's contractor, requesting assistance with a water quality issue
at Hope Elementary School. Erickson-Hall indicated that recent samples t.hat were taken at the
school showed higher levels of copper than would normally be found in potable water. CMWD
staff immediately responded to the school and began investigating.
Discussion
Upon arrival at Hope Elementary School on Feb. 24, 2022, CMWD staff took a water quality
sample in one of the classrooms and found the TDS1 results of the water to be 740 milligrams per
liter (mg/I), which confirmed that the water coming into the school's potable water system likely
included recycled water.
CMWD staff immediately shut-off the CMWD potable water supply to the campus. Once the
potable water was shut-off, water was still reaching the classrooms throughout the school
property. This further confirmed that the recycled water lines had been inappropriately
connected to the potable water system within the school campus. The campus is under active
construction.
CMWD staff notified the school principal that all water supply to the campus would be shut-off
immediately. Without water service on campus, the school sent home students and staff. CMWD

staff notified the County Department of Environmental Health and Quality, or DEHQ, and the
State Water Resources Control Board of the cross-connection and began to work with staff from
these two agencies to ensure that all needed steps were taken and regulations met.
During the investigation of the school site and its water pipes on Feb. 24, 2022, CMWD staff
found newly constructed PVC pipes with a recycled water pipe connected to the CUSD1s potable
water line behind one of the classrooms located at the back corner of the school property.
CMWD staff eliminated the cross-connection by disconnecting the recycled water line from the
potable water system. Staff also removed the school1s recycled water meter.
After the school was vacated, CMWD staff began introducing chlorine into the distribution
system to disinfect the potable water lines. This process involves injecting chlorine, or flushing,
and follows a nationally recognized standard for disinfecting potable water mains. CMWD staff
performed this process from Friday to Saturday, Feb. 25-26, 2022. CMWD staff took two sets of
samples and sent them to a laboratory for testing ..
Both sets of samples showed that the potable water met water quality standards. CMWD staff
then contacted the State Water Board and notified the CUSD that the school could reopen. The
CUSD sent notification to the families of the students stating that the potable water meets all
standards and the school would reopen on Monday, Feb. 28, 2022.
Recycled water is not connected at the school site at this time. Prior to reinstallation of the
recycled water meter and restoration of recycled water service to the school site, the CMWD will
require the CUSD to submit a recycled water use site plan that must receive approval from the
CMWD, the DEHQ and the State Water Board.
While CMWD staff do not know how long the recycled water was in the potable water system at
the school, CUSD staff told CMWD staff that there had previously been multiple complaints from
food services, teachers and students starting in approximately August 2021. The CUSD also told
the CMWD that its construction contractor sampled the water on the following dates:
April 2, 2021, Aug. 11, 2021, Nov. 10, 2021, Jan. 11, 2022, and Feb. 23, 2022. The contractor
tested those samples for E.coli, which was not detected. The contractor continued to operate
the system until Feb. 24, 2022, when the construction contractor contacted the CMWD.
Next Steps
CMWD staff will continue to address the issue by working with the CUSD and others and taking
the following actions:
1.State Water Board Incident Report
CMWD staff must provide a final incident report to the State Water Board by
March 16, 2022. The State Water Board may grant an extension, if necessary.
2.Communication
The CUSD published a "Hope Elementary School Water Issue Frequently Asked
Questions11 document dated Feb. 28, 2022, on its website, which is provided as

Attachment A. This document answers questions and addresses concerns shared during
calls to the CUSD from the school's families. CMWD staff are assisting with phone calls as
needed.
Two TV news stations, channels 10 and 7, covered this issue after being notified by
concerned parents.
The CMWD and other city departments including Construction Management &
Inspection, Community Development, and Communication & Engagement worked
together to answer the reporters' questions and explain the city's role. See attachment B
for the information provided to the media.
3.CMWD Testing at Other CUSD Schools
CMWD staff will work with the CUSD and the DEHQ to schedule cross-connection testing
at the other schools that have recycled water and projects under construction. This will
determine whether any changes have been made to those water systems and whether
the potable and recycled water being served at these other schools meet all applicable
regulatory standards.

On Thursday, February 24, the City of Carlsbad was notified following a suspicion that the domestic water lines
at Hope Elementary were crossed with reclaimed water irrigation lines on campus. Due to this suspicion, all
water was immediately shut off on campus, and students were sent home.
The District had been working with its contractors to investigate reported concerns with intermittent
discoloration of the water at Hope Elementary. Water samples were taken throughout the campus and tested
for safety. No pathogens (such as e.coli) were found in the samples, and the water was found to be within safe
drinking standards. In an abundance of caution, we made bottled water available to students and staff while we
looked further into the problem.
Additional testing conducted by the City confirmed the suspicion. The City of Carlsbad worked closely with the
district to locate the problem and repair it immediately.
The City of Carlsbad gave the clearance to reopen on Monday, February 28, after treating the school's
plumbing and conducting water quality tests, which were analyzed by a third-party laboratory for safety.
After the cross connection was removed, the City purged the plumbing system throughout the school with
highly chlorinated water, and then allowed the chlorinated water to sit overnight. On Friday, City crews then
flushed out the chlorinated water with potable water. The water tests were conducted after this process was
completed. We are grateful for the City's assistance in helping us resolve this problem.
Additionally, we replaced all of the drinking water dispenser filters on campus.
Frequently Asked Questions
How did the cross connection occur?
We are still investigating how this occurred. It appears that this happened during the course of construction
based on the timing of complaints and the location of the cross connection.
When did the complaints begin?
Shortly after the school reopened in the fall, there were sporadic reports of water discoloration in limited
locations; the construction teams indicated that the issue was aesthetic and would resolve upon flushing the
system.
We received additional complaints about intermittent problems with the water color, taste, and smell in
November 2021. An investigation was conducted with testing results described below that did not indicate the
water was unsafe.
Following additional complaints after winter break, more tests were performed and the District hired an
independent consultant to look deeper into the cause of the recurring problem.

How did the District respond?
The District notified its contractors of the problem and brought them onsite to diagnose the issue. Initially it
was believed that the source was contamination that was introduced during construction or other disturbance
in the infrastructure which would resolve with flushing the plumbing lines. The water testing described below
was performed to rule out harmful bacteria, but testing was not ordered that would have detected the higher
levels of chlorine, water hardness, and alkaline content that is found in reclaimed water.
The testing information we received concerning the safety of the water was shared with the principal, and she
was relying on the same information provided to the District.
As complaints persisted, the contractors were brought out again and eventually the District hired an
independent consultant to conduct additional testing to determine the problem.
Was the water tested?
Yes. Testing was performed by the plumbing contractor. The water was tested twice before the school
reopened-once before opening the new kindergarten classroom that was being temporarily used as an
administration office, and once before the entire school was reopened.
The water was tested for e.coli and other coliforms which would indicate harmful bacteria in the water.
Reclaimed water is disinfected, which is why the tests did not detect those substances.
Following school opening, tests were again conducted by the contractor while the problem was being
investigated. Again, no e.coli or other coliforms were detected.
Following the test in January, the District hired an independent consultant to determine the cause of the
ongoing complaints. The increased levels of chlorine, water hardness, and alkaline content indicated the cross
connection.

Was the problem school wide or confined to specific locations?
The plumbing system on the campus was being served by both potable water and water from the cross
connection. The contaminated water mixed with the potable within the plumbing lines.
What contaminants might students have been exposed to?
Reclaimed water is highly treated and disinfected, but still contains higher levels of salts, dissolved minerals,
nitrogen, and phosphorus than drinking water. The EPA has stringent guidelines to keep biological
contaminants to a safe minimum.
Do students need to be tested by their doctor?
The State Water Resources Control Board advises parents to seek medical attention if your child exhibits
symptoms of stomach or intestinal illness.

What was done to restore drinking water to the school?
All water to the school was turned off while we located the cross connection on Thursday. February 24. Once
the connection was found, it was removed and a new connection was made to the potable water.
City of Carlsbad crews then purged the plumbing system with heavily chlorinated water on Thursday night.
That water remained in the system overnight and was then flushed out on Friday. Following the flushing
process, the City conducted its own water sampling on Friday (February 25) and Saturday (February 26). The
County of San Diego reviewed the test results and gave clearance for the school to reopen on Monday
(February 28).
Is this a problem at other schools that had recent construction?
We have not had any reports, but will work with the City of Carlsbad to conduct cross connection testing at
Kelly and Magnolia as a precaution.
Why didn't the District notify parents of the complaints?
We are sincerely sorry that this was not reported earlier. We relied on the reports and information from our
contractors, construction and bond program managers that the water had been tested for safety, that the issue
was aesthetic, and would resolve. We did not anticipate the cause of the problem, and we regret that we did
not discover the cause sooner. Once we were alerted to the cause, we took all the steps necessary to prevent
further exposure; to inform our community; and to resolve the problem.

The Carlsbad Unified School District's construction contractor contacted the Carlsbad Municipal Water
District last Thursday, Feb. 24, to ask for help identifying the cause of water quality concerns at Hope
Elementary.
We sent staff to the school right away and confirmed that a school recycled water pipe had been connected
to one if its regular water pipes.
We notified the State Water Resources Control Board and the County of San Diego Department of
Environmental Health and Quality, which is the protocol when a "cross-connection" has been identified.
Water district staff has worked with the school district and its contractor to locate the affected pipes and
oversee the disconnection of the recycled water cross connection. Once that was done, water district staff
followed the state's protocol for flushing and disinfecting the campus water system, which included additional
testing.

The school district didn't submit plans to work on its recycled water system. That is what would have
triggered an inspection by the city.

CROSS-CONNECTION
INCIDENT REPORT
1.0 Introduction
This report provides information related to a recycled water cross-connection
discovered at Hope Elementary School (Hope School), located at 3010 Tamarack
Avenue, Carlsbad, CA and the Carlsbad Municipal Water District’s (CMWD) subsequent
response.
2.0 Cross-Connection Discovery
On Thursday, February 24, 2022, at approximately 8:00 a.m., CMWD received a call from
Tyler Ennis, Project Manager for Erickson-Hall, Carlsbad Unified School District’s (CUSD)
construction contractor, requesting assistance with a water quality issue at Hope
School. Mr. Ennis indicated that recent samples taken at the school showed higher
levels of copper than would normally be found in potable water. CMWD immediately
responded to the school and began an investigation.
At approximately 8:25 a.m., CMWD arrived on the school property. A chronology of the
events that followed are summarized in the following section.
3.0 Enrollee’s Response to Cross-Connection (Chronologic Narrative of Actions)
February 24, 2022 (unless otherwise noted)
3.1 Upon arrival at Hope School, CMWD staff met with Derrick Anderson, CUSD’s
Construction Director, to discuss the reported water quality sampling done by
Erickson-Hall that showed elevated levels of constituents. CMWD immediately
took new baseline total dissolved solids (TDS) samples at the potable water
backflow prevention device (result: 440 mg/l concentration) and the recycled
irrigation system quick coupler (result: 900 mg/l concentration). Mr. Anderson
then showed CMWD containers of water samples in Classroom 34 CUSD had
previously collected at an unknown time. (Photos 1 and 2).

3.2 CMWD tested a water sample from the classroom faucet. The water was
discolored and odorous. CMWD’s test showed a TDS concentration of 740 mg/l,
significantly higher than measured in the baseline potable water sample, which
confirmed the school’s potable water system was likely contaminated with
recycled water. CMWD shut off the potable water supply at the public meter, yet
water was still reaching classrooms through the school’s private water system.
This further confirmed a cross-connection existed.
3.3 CMWD closed the public potable water supply connection at the CMWD meter
and notified the San Diego County Department of Environmental Health and
Quality (County DEHQ) and the State Water Resource Control Board, Division of
Drinking Water (State DDW) of the CUSD plumbing system cross-connection.
CUSD closed the campus and students, teachers and other staff were sent home
for the remainder of the day.
3.4 CMWD began investigating the private plumbing systems on the campus with
the assistance of CUSD, Erickson-Hall, and its plumbing subcontractor. CMWD
discovered a flexible hose connection at the back of Classroom 35 (Photo 3
below) which connected the irrigation line to the potable water line. Later in the
day, as the investigation continued, CMWD requested that the CUSD plumbing
subcontractor disconnect the hose (Photo 4 below).

3.5 While investigating the site, a plumbing subcontractor informed Eric Sanders,
CMWD Utilities Manager, that the plumbing subcontractor had made the
connection.
3.6 CMWD does not know the exact date the cross-connection occurred. However,
CUSD indicated the construction contractor began painting the pipes in Photo 3
white around July 7, 2021 and the flexible hose was installed sometime after the
painting.
3.7 Additionally, on February 25, 2022, Mr. Anderson told CMWD that food services
staff, teachers and students began complaining about water quality in August
2021. Mr. Anderson sent an email to CMWD that includes a complaint of yellow
water and residue from the principal, Ms. Rossiter. The same email chain also
indicates that Erickson-Hall sampled and tested the water on April 21, 2021,
August 13, 2021, September 10, 2021, and January 11, 2022 (Attachment A). -
The test reports (Attachment B) provided by CUSD indicate the testing actually
took place on March 31, 2021, August 11, 2021, November 9, 2021, and
January 11, 2022. All four samples were tested for E. coli, which was not
detected. The report from January 11, 2022 indicates that the sample was tested
for other analytes normally present in recycled water and several were detected.

The school continued to operate its private plumbing and irrigation systems until
February 24, 2022, when CUSD contacted CMWD.
3.8 State DDW, County DEHQ and CMWD, with the cooperation of CUSD and its
contractors, began working together to correct the situation. The State DDW
prescribed a course of action (Attachment C), which CMWD and County DEHQ
followed with cooperation from CUSD and Erickson-Hall.
3.9 CMWD reviewed the Recycled Water Use Site Plan approved for the school site
when County DEHQ and CMWD approved the conversion of the school’s
irrigation system to recycled water in 2017. Neither CUSD nor its construction
contractor submitted plans to the City or County DEHQ for approval of
modification of the school’s recycled water irrigation system as part of CUSD’s
current construction activities.
3.10 On February 24, CMWD requested plans for the school’s plumbing system. On
February 25, Mr. Ennis from Erickson-Hall provided five drawing sheets: A111-
Overall Site Plan, A124-Enlarged Site Plan New Classroom Building, P101-
Plumbing Site Plan, C005-Site Utilities Plan, C006-Site Utilities Plan, and C007-
Site Utilities Plan (Attachment D), all approved in June 2020 by the Division of
the State Architect for the new construction. None of the sheets show
modifications to the school’s recycled water system. In addition, the back area
of the school site where the cross-connection occurred is not depicted on the
drawing sheets. While drawing sheet C005 shows a 2” irrigation line running
under one of the new classroom buildings, Building H, CUSD did not provide
plumbing plans for relocation of this irrigation line, which had been relocated.
3.11 CMWD performed an initial test of the potable water backflow prevention
device to confirm proper operation of the device. The test certification for the
potable water backflow prevention device was submitted by email to State DDW
on February 24, 2022 (Attachment E). County DEHQ completed its report on
February 27, 2022 (Attachment F).
3.12 CMWD staff installed a pressure recorder on the potable water backflow
prevention device, test cock #4, with the potable water supply off and the
recycled water supply remaining on, to confirm a recycled water cross-
connection. A rise in pressure was observed at the recorder, verifying the cross-
connection was reaching the entire school property.

3.13 CMWD directed CUSD to physically sever the cross-connection previously
identified behind Classroom 35 and instructed the CUSD contractor mark the
recycled water pipe in purple. (Photo 5).
3.14 CMWD, under the direction of State DDW, and with County DEHQ oversight,
performed part one of a 2-way test, which is a 4-hour test of the school’s private
water system.
3.15 As ordered by State DDW, CUSD sent an email notification of school closure for
Friday, February 25, 2022 (Attachment G).
3.16 CMWD staff flushed and began introducing chlorine into the school’s potable
plumbing system in accordance with standard AWWA C651. CMWD used the
slug method with 100 mg/l concentration of chlorine as a disinfectant which is to
remain in the pipe for at least three hours. The chlorinated water remained in
the private plumbing system for ten hours and averaged at least 100 mg/l of free
chlorine. The water in the private plumbing system was then thoroughly flushed
and tested periodically for chlorine residual until a total chlorine residual of
normal system potable water, approximately 3 mg/l, was reached.

3.17 Following the disinfection procedure, CMWD returned to the school and
confirmed the chlorine residual at each faucet and flushed the super-chlorinated
water. After normal system water was introduced to the school’s plumbing,
CMWD took the first round of samples for bacteriological testing from 11 sites
throughout the school as shown on the map of the Hope School buildings and
sampling locations (Attachment H). After 24 hours, and after receiving results

showing the absence of E. Coli bacteria in the first round of samples, CMWD took
the second round of samples and tested them on February 26, 2022. CMWD also
directed CUSD to replace the filters at all drinking fountains, which CMWD
oversaw.
On Sunday, February 27, the test results for the second round of samples came
back absent for E. Coli bacteria. CMWD communicated this information to State
DDW, and State DDW indicated the water in the school plumbing was safe.
CMWD subsequently submitted the test results to County DEHQ and State DDW
(Attachment I).
3.18 CMWD performed, under the direction of State DDW, and as part two of the 2-
way test, a 24-hour test on the recycled water irrigation system. Upon
completion of the test on February 26, the test report was sent to County DEHQ.
The recycled water meter was removed and the meter valve was locked off by
CMWD (Photo 6). There has been no recycled water service to the school site
since that time.
3.19 On Sunday, February 27, 2022, State DDW approved the corrective actions taken
and deemed the water quality test results acceptable and granted approval to
reopen the school (Attachment J).
3.20 CUSD reopened the school on Monday, February 28, 2022 (Attachments K) and
provided information about the incident to the parents and staff (Attachment L).

3.21 CMWD staff have continued to work with CUSD and CUSD’s construction
contractor to expose the school’s recycled water irrigation system and assure
that all irrigation piping is separate from any other system. No additional cross-
connection locations have been found.
3.22 The meter that previously served recycled water to this site has been removed
and the school has no connection to the recycled water system at this time.
Hope School will not be allowed to connect back to the recycled water system
until CUSD submits landscape and irrigation plans to CMWD and County DEHQ
for approval. On March 10, 2022, CMWD provided CUSD and CUSD’s
construction contractor the following steps that must be taken prior to
reconnection of the school’s recycled water irrigation system.
3.22.1 CUSD’s construction contractor, under the direction of CUSD, must
excavate areas identified by CMWD and County DEHQ staff to verify the
potable water line cross-connected with irrigation system is fully
abandoned at the point of connection(s).
3.22.2 CUSD will submit current irrigation plans for the entire school site,
along with an E-27 water meter permit application to the city’s Land
Development and Engineering Department. These plans must include a
temporary potable high line as well as any alterations to the school’s
recycled water irrigation system.
3.22.3 Once routed to CMWD via the Land Development and Engineering
Department, the plans will be reviewed by CMWD and County DEHQ.
3.22.4 The old potable irrigation service still on site will be utilized as the feed
for the temporary potable high line to irrigation system.
3.22.5 CUSD must instruct its construction contractor to expose all piping
around the current recycled meter box and the old potable irrigation
meter box, including the two-inch backflow still on site.
3.22.6 The two-inch backflow on the old potable irrigation line will be tested
by CUSD and witnessed by CMWD and must pass in order for a meter
to be issued. If the backflow test does NOT pass, a new device must be
installed.

3.22.7 After the backflow testing and the connection between the irrigation
line and the old potable irrigation line is made and observed with
CMWD cross-connection oversight, potable water can be introduced to
the site utilizing this connection.
3.23 Future connection to recycled supply for irrigation:
3.23.1 After approval of new landscape and irrigation plans by County DEHQ and
CMWD, CMWD, along with County DEHQ oversight, will then schedule
and perform a cross-connection test for the site.
3.23.2 After successful completion of the cross-connection test, a recycled
meter can be issued and installed on site, and CUSD can disconnect the
temporary potable feed, with CMWD oversight, and reconnect the
existing irrigation to the recycled water meter.
4.0 Hope School Recycled Water Use Site Certification/Site Utilities Plan
4.1 On April 7, 2017, County DEHQ issued a Recycled Water Use Site Certification,
LRWS 2015-359, following successful completion of cross-connection testing in
accordance with State, County and CMWD regulations governing the use of
recycled water. At that time, portions of the irrigation system for Hope School
were converted from potable to recycled water supply.
4.2 This 2017 certification and the CMWD Rules and Regulations for Recycled Water
Use, which are provided to the water user, such as CUSD, upon site certification,
require the designation of a Site Supervisor. The Site Supervisor is responsible for
the operation, maintenance or alteration of the on-site recycled water irrigation
system, including the requirement to submit plans for review and approval by
CMWD and County DEHQ before making any changes to the on-site recycled
water system.
4.3 The Hope School Site Utilities Plan relating to the current construction activities
at Hope School was approved by the Division of State Architect in June 2020 and
construction at Hope School was underway in the summer of 2020.
4.4 The city issued a Stop Work Order to CUSD on July 30, 2020 because the city had
not received an application for a grading permit for grading work being
performed at Hope School. Around this same time period, CMWD reminded
CUSD that any changes to the school’s recycled water irrigation system would
require approval from CMWD and County DEHQ. Although CUSD sent an email

indicating that it did not believe its construction related plumbing activities
implicated the school’s recycled water system (Attachment M), CMWD
recommended CUSD install a temporary potable high line for the irrigation
system to avoid any potential for a cross-connection (Attachment N).
4.5 On February 24, 2022, when CMWD responded to Hope School, construction at
the school was proceeding despite the Stop Work Order that had been issued by
the City. CUSD or its contractor had not submitted landscape and irrigation
plans to the city, which are reviewed and approved by CMWD and the County
DEHQ before the installation of, or modifications to a recycled irrigation system.
4.6 As described above in Section 3.10, CUSD’s construction contractor recently
provided the city with five drawing sheets. No modifications to the school’s
recycled water irrigation system are shown on these drawings. While drawing
sheet C005 shows a 2” irrigation line running under one of the new classroom
buildings, Building H, irrigation plans have not been submitted to CMWD. The
irrigation line under Building H has been relocated. The cross-connection
location was not on the relocated line but in an area not shown on the drawings.
5.0 Recommended Actions
Additionally, CMWD will perform the following actions:
5.1 Schedule cross-connection with County DEHQ for other CUSD schools served by
CMWD that have recycled water irrigation systems and have undergone recent
construction work on plumbing or irrigation systems to ensure compliance with
regulatory standards.
5.2 Modify CMWD Recycled Water Rules and Regulations to assign Site Supervisor
status to a person in a management or executive management position of
authority in the water user entity for compliance with potable and recycled
water regulations.
5.3 Request assistance from the State DDW and County DEHQ to reinforce CMWD’s
authority over recycled water regulations at school sites.

Please see the attached report. I reached out to Gary at Interpipe one more time to make sure I had the correct
information. I tried to organize the info in the best way. Here is the summary to date, findings and some
recommendations:
We performed a 4th test with results on 1/11/22. Testing history as follows.
1.4/21 preliminary test for E.coli & Bac‐T
2.8/13 Testing as required for turnover for E.Coli & Bac‐T
3.9/10 Testing in response to concerns from the school for E.Coli & Bac‐T
4.1/11 (attached)‐ most recent test further investigating. Includes E.Coli & Bac‐t as well as Odor and Color.
The most recent test still shows the water is within standards for safe drinking water (e.coli and Bac‐t).
Prior to the final test, our team performed multiple visual inspections and building flushing in early December at
multiple locations and prior to the most recent test. Results were mixed. In certain buildings water was clear and upon
return on a different day, would have faint traces of color then clear again on another day.
Our field investigations have narrowed the potential source location to the North side of the campus. Specifically
the main that runs by Building A and west between buildings F & J The South side showed no visual traces of color.
The work that was performed in the area above was to an existing main pipe. Our connection points were existing
copper. We assume the entire main (existing portions to remain) heading East/West was copper as well. Per plans we
installed new PVC pipe to parts of the existing copper pipe to modify the main for the new buildings.
There is an existing vault somewhere by the drive that was not part of this project (I personally have not seen this
vault). We believe this is the source for the domestic water. We did not check anything there.
When reviewing the most recent test results, it appears the color/odor may be coming from trace metals found in
the report. This is only an observation that we are making. We recommend the report is reviewed with someone familiar
with EPA regulation requirements in this regards.
We have not seen or heard reports of ground water coming to the surface in the suspect area that could be an
indication of a water break.

Recommendations
o Find out if there are any anomalies in water usage and or after hour meters running to see if there
could be a water main break
o Check with the city to see if the incoming water has issues.
o Understand what is in the existing vault at the drive area to see if there could be something there?
o Look at replacing the old copper mains with PVC.

Carlsbad MWD must take the following additional actions:
1.Submit plan for how students’ parents will be notified. If there have been any visitors to the school, a plan to
notify them as well must be submitted. Submit copy of notification language to DDW for approval.
2.Submit test cert for RP device from 2/24/2022 test by COB today 2/24/2002.
3.Flushing, disinfection and bacteriological confirmation testing of the onsite plumbing following AWWA pipeline
disinfection procedures – submit results to DEHQ and DDW
4.Submit as‐built use site plans (PW & RW properly annotated) clarifying what plumbing changes have been made,
both that resulted in the cross connection and the necessary plumbing corrections.
5.2‐way full cross connection test must be performed (to be witnessed by DEHQ).
6.Confirm that the school will remain closed (or alternative water provided for all human consumption purposes)
until DDW approves of the corrective actions taken and water quality results to reopen the school.
7.Incident report by March 4th, 2022. Include documentation for the initial shutdown testing performed when RW
was activated for the use site.

Shutdown test description:
This cross connection inspection was required following the discovery of a cross connection at Hope
Elementary School. School personnel notified the Carlsbad Municipal Water District of concerns
regarding the color and odor of the water. The water district and school construction team identified a
single point where the recycled water system was directly connected to the potable system. The
connection was separated and this cross connection inspection was conducted to determine if that
point was the only cross connection between the two systems.
The last cross connection inspection was conducted on April 5, 2017 and was witnessed by DEHQ
staff. During the April 2017 inspection, no cross connections were observed or detected.
Two clocks control the irrigation system. Clock A was equipped to operate 16 stations and clock B
had 11 stations. In addition, 2 valve boxes were identified on the southwest side of the property that
controlled irrigation on the exterior of the school fencing (southwest portion of property). Each valve
was operated for 2 minutes.
At the end of the 4 hour test the following was observed:
• Gauge pressure dropped gradually decreased from 22 psi to 10 psi.
• No pressure spikes observed during the test.
• The system re-pressurized to approximately 92 psi once water was restored.
The reverse pressure differential test was conducted, taking place from February 25, 2022 – Feb 26,
2022. Pressure gauges were connected to five test points on the recycled water system, and pressure
was dropped to approximately 20 psi. The potable system was in operation during this time at full
pressure. The gauges were left to track for approximately 22 hours which is standard with reverse
testing. No spikes in pressure were observed on the test charts submitted to DEHQ.
Unpermitted modifications to the recycled and potable system have been made to this site. Hope
Elementary School shall submit as built plans of the site for review and approval by DEHQ. The
Carlsbad Municipal Water District has pulled the recycled water meter for DEH2015-LRWS-000359. The
recycled water meter shall not be approved for connection until all plans are approved by DEHQ, site
visits are performed and a final cross connection inspection occurs where DEHQ is present.

For Hope families
Dear Hope Families,
This is a follow up to my earlier message regarding the school closure today. Hope will remain
closed tomorrow, Friday, 2/25/22. We will not shift to online learning for this day.
Today, the City of Carlsbad was notified following a suspicion that the domestic water lines were
crossed with reclaimed water irrigation lines on campus. Due to this suspicion, all water was
immediately shut off on campus, and students were sent home.
The District had been working with its contractors to investigate reported concerns with
intermittent discoloration of the water at Hope Elementary. In an abundance of caution, we
made bottled water available to students and staff while we looked further into the problem.
Additional testing conducted by the City confirmed the suspicion. The City of Carlsbad is
working closely with the district. We have located the problem and are repairing it
immediately. Following the repair, all water lines on campus will be flushed with a high dosage
of chlorinated water before restoring normal water services. The City of Carlsbad will conduct
water samples and submit them to a laboratory to ensure that the water meets safe drinking
standards. Multiple tests will be conducted over the weekend. We appreciate the City’s support
as we work together to reopen the campus safely.
We are hopeful we will be able to resume school on Monday, 2/28/22, but need to wait until we
have clearance from the City based on the laboratory tests over the weekend. I will send an
additional communication no later than 7pm Sunday about the status of school on
Monday.
We apologize for any inconvenience this school closure causes, however student and staff
safety is always the top priority of school and district staff.

I understand the following:
1.Carlsbad MWD and Hope Elementary have disconnected the cross connection that was discovered
last week.
2.The use site’s PW meter is protected by an RP device, which was retested by the day of the cross
connection discovery. Test cert is attached. The site is served by only 1 RW meter and only 1 PW meter.
3.A 2‐way shutdown test has been performed with DEHQ oversight (reports and DEHQ test report
attached).
4.The RW meter has been removed and the irrigation system served by RW is currently dry. CMWD will
require Hope Elementary to submit RW use site plans and receive CMWD, DEHQ and DDW approval
prior to restoring RW service to the use site. In addition, if the existing RW system is to be highlined in
the interim, CMWD will require the highline to be protected by an RP device and that device will be
tested after installation and must receive a passing test.
5.The onsite potable water plumbing has been disinfected, flushed and tested per AWWA C651‐14.
Two sets of bacti’s were collected 2/25 and 2/26 from 11 sites, covering the majority of all buildings on
the campus. Both reports and the sample siting plan are attached. The results are acceptable.
Based on the actions taken by CMWD and acceptable bacti results, CMWD may lift the water quality
advisory. Great job to you and the CMWD staff for the swift and effective response. As previously
discussed, please submit the incident report no later than 3/4/2022.

Dear Hope Elementary Families,
District staff shared the following message with me a few minutes ago:
Hope Elementary School is cleared to reopen tomorrow, Monday morning, at its usual start

The City of Carlsbad has given the clearance to reopen, after treating the school’s plumbing and conducting water quality tests on Friday and Saturday which were analyzed by a third-party
laboratory.
After the cross-connection was removed, the City purged the plumbing system throughout the
school with highly chlorinated water, and then allowed the chlorinated water to sit overnight. On Friday, City crews then flushed out the chlorinated water with potable water. The water tests
were conducted after this process was completed. We are grateful for the City’s assistance in
helping us resolve this problem.
Additionally, we replaced all of the drinking water dispenser filters on campus.
While we are still investigating how this happened, it appears to us that the water lines were crossed during construction before the campus reopened in August. The water had been tested
by the contractor using a standard test for safety to check for e. Coli and other bacterias before
the school reopened, with negative results; but the tests did not detect the cross-connection. Subsequent tests by our contractors used the same method, with the same
results.
As mentioned last week, the state water authorities recommend that if your child has any
symptoms of stomach or intestinal illness, please consult with your physician.
We’re sorry that this situation occurred, and regret the concern, inconvenience, and frustration for our students, families, teachers and staff.
I look forward to welcoming our Hawks back to campus tomorrow, Monday, 2/28/22.

On Thursday, February 24, the City of Carlsbad was notified following a suspicion that the domestic water lines
at Hope Elementary were crossed with reclaimed water irrigation lines on campus. Due to this suspicion, all
water was immediately shut off on campus, and students were sent home.
The District had been working with its contractors to investigate reported concerns with intermittent
discoloration of the water at Hope Elementary. Water samples were taken throughout the campus and tested
for safety. No pathogens (such as e.coli) were found in the samples, and the water was found to be within safe
drinking standards. In an abundance of caution, we made bottled water available to students and staff while we
looked further into the problem.
Additional testing conducted by the City confirmed the suspicion. The City of Carlsbad worked closely with the
district to locate the problem and repair it immediately.
The City of Carlsbad gave the clearance to reopen on Monday, February 28, after treating the school’s

Frequently Asked Questions
How did the cross connection occur?
We are still investigating how this occurred. It appears that this happened during the course of construction
based on the timing of complaints and the location of the cross connection.
When did the complaints begin?
Shortly after the school reopened in the fall, there were sporadic reports of water discoloration in limited
locations; the construction teams indicated that the issue was aesthetic and would resolve upon flushing the
system.
We received additional complaints about intermittent problems with the water color, taste, and smell in
November 2021. An investigation was conducted with testing results described below that did not indicate the
water was unsafe.
Following additional complaints after winter break, more tests were performed and the District hired an
independent consultant to look deeper into the cause of the recurring problem.

How did the District respond?
The District notified its contractors of the problem and brought them onsite to diagnose the issue. Initially it
was believed that the source was contamination that was introduced during construction or other disturbance
in the infrastructure which would resolve with flushing the plumbing lines. The water testing described below
was performed to rule out harmful bacteria, but testing was not ordered that would have detected the higher
levels of chlorine, water hardness, and alkaline content that is found in reclaimed water.
The testing information we received concerning the safety of the water was shared with the principal, and she
was relying on the same information provided to the District.
As complaints persisted, the contractors were brought out again and eventually the District hired an
independent consultant to conduct additional testing to determine the problem.
Was the water tested?
Yes. Testing was performed by the plumbing contractor. The water was tested twice before the school
reopened–once before opening the new kindergarten classroom that was being temporarily used as an
administration office, and once before the entire school was reopened.
The water was tested for e.coli and other coliforms which would indicate harmful bacteria in the water.
Reclaimed water is disinfected, which is why the tests did not detect those substances.
Following school opening, tests were again conducted by the contractor while the problem was being
investigated. Again, no e.coli or other coliforms were detected.
Following the test in January, the District hired an independent consultant to determine the cause of the
ongoing complaints. The increased levels of chlorine, water hardness, and alkaline content indicated the cross
connection.

Was the problem school wide or confined to specific locations?
The plumbing system on the campus was being served by both potable water and water from the cross
connection. The contaminated water mixed with the potable within the plumbing lines.
What contaminants might students have been exposed to?
Reclaimed water is highly treated and disinfected, but still contains higher levels of salts, dissolved minerals,
nitrogen, and phosphorus than drinking water. The EPA has stringent guidelines to keep biological
contaminants to a safe minimum.
Do students need to be tested by their doctor?
The State Water Resources Control Board advises parents to seek medical attention if your child exhibits
symptoms of stomach or intestinal illness.

What was done to restore drinking water to the school?
All water to the school was turned off while we located the cross connection on Thursday. February 24. Once
the connection was found, it was removed and a new connection was made to the potable water.
City of Carlsbad crews then purged the plumbing system with heavily chlorinated water on Thursday night.
That water remained in the system overnight and was then flushed out on Friday. Following the flushing
process, the City conducted its own water sampling on Friday (February 25) and Saturday (February 26). The
County of San Diego reviewed the test results and gave clearance for the school to reopen on Monday
(February 28).
Is this a problem at other schools that had recent construction?
We have not had any reports, but will work with the City of Carlsbad to conduct cross connection testing at
Kelly and Magnolia as a precaution.
Why didn’t the District notify parents of the complaints?
We are sincerely sorry that this was not reported earlier. We relied on the reports and information from our
contractors, construction and bond program managers that the water had been tested for safety, that the issue
was aesthetic, and would resolve. We did not anticipate the cause of the problem, and we regret that we did
not discover the cause sooner. Once we were alerted to the cause, we took all the steps necessary to prevent
further exposure; to inform our community; and to resolve the problem.

Per my site visit on Thursday August 13, 2020, I had a conversation with you and Dereckabout the recycled irrigation and my concerns with the changes being made.
I suggested to Derek, It would be in the best interest of the City of Carlsbad Municipal WaterDepartment and CUSD that we high-line Hope Elementary off the old existing potable
irrigation service temporarily until the completion of Hope Elementary's renovations, at whichtime we will conduct a full cross connection on entire site with DEH over-site on approved
recycled irrigation plans, Derek is in agreement.
The next step would be getting your temporary water meter, which can be done by contactingEngineering and Land Development at (760) 602-2750. There is $900 (refundable) deposit
and a $192 (non-refundable) rental charge for the meter.
