To Whom It May Concern: The purpose of this letter is to serve as the Permanent Structural BMP Construction Self Verification Form for
the permanent structural BMPs within the Carlsbad Senior Housing project. This certification specifically applies to the following project:
Project Name: Carlsbad Senior Housing Report Title: Priority Development Project (PDP) Storm Water Quality Management Plan (SWQMP) for Carlsbad Senior Housing, Steadfast Companies (Final Engineering) Report Date: August 2, 2019 City of Carlsbad Project ID #: MS2017-0004 DWG #: DWG 516-7A Rick Engineering Company Job Number: 17759-B This certification form specifically includes the BMP:
• Biofiltration Basin o BMP 2 The intent of this letter is to certify that observations have been made at the completion of the BMP and the BMP is in-place and that it appears to match the approved Storm Water Quality Management Plan (SWQMP)
and construction documents with respect to the following, as applicable: type of BMP, location, size, footprint, depth, and condition (i.e. – operational).
