Airosmith Engineering has been requested to perform a structural analysis on the existing 42 ft. Mono-
Eucalypt us for the Verizon Wireless proposed install. The structure was analyzed using RISA-3D Version
19.0 analysis software. Selected output from the analysis is included in this report.

Upon reviewing the results of this analysis, it is our opinion that the existing structure meets the specified
code requirements. The 42' SCI Mono-Eucalyptus structure and foundation are considered acceptable to
support the final loading configuration as listed within in this report. The controlling structure and
foundation usages are displayed in the tables below:

based on geotechnical and foundation information, therefore no modification or reinforcement of the

Passing Mount Analysis requires a PMI due to a modification in loading.

confirmation that the installation was completed in accordance with this Passing Mount -
Analysis.
• Contractor shall relay any data that can impact the performance of the mount, this indu.des

Base Requirements:

• Contractor shall ensure that the safety climb wire rope is supported and not adversely
impacted by the install of the modification components. This may involve the install of wire
rope guides, or other items to protect the wire rope. If there is conflict, contact the SMART Tool
engineer for recommendations.

1. Contractor to install (1) new OVP pipe, 36" long P2 STD into the right standoff horizontal (as seen from
behind the mount) in alpha sector. Connect to standoff at 15" from standoff end connected to
monopole using crossover plate (VZWSMART-MSK6). Tip of OVP pipe shall be at 27" above the standoff.
Install (1) proposed OVP onto the new OVP pipe at 12" from top of pipe. (Total of (1) 36" long P2 STD
pipe and (1) VZWSMART-MSK6).
